肥胖儿童高尿酸血症与胰岛素抵抗、血脂的相关分析

摘　　要：目的探讨肥胖儿童血清尿酸(UA)与胰岛素抵抗、血脂的相关性。方法选取本院655例肥胖儿童设为观察组,另选取同时期655例健康非肥胖儿童作为对照组,分别检测两组儿童血清UA、空腹血糖(FPG)、空腹胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C),并计算胰岛素抵抗指数(HOMA-IR)。再按照血清UA水平将观察组分为高UA血症组(HUA组)和正常UA血症组(NUA组)。结果观察组血清UA、FPG、FINS、TC、TG、LDL-C,HOMA-IR显著高于对照组,血清HDL-C显著低于对照组,差异有统计学意义(P<0.05)。HUA组血清FPG、FINS、TG,HOMA-IR显著高于NUA组,差异有统计学意义(P<0.05),HUA组与NUA组血清TC、HDL-C、LDL-C相比差异无统计学意义(P>0.05)。经Pearson直线相关分析,肥胖儿童血清UA与FINS、TC、TG、LDL-C,HOMA-IR呈显著正相关,而与血清HDL-C呈显著负相关(P<0.05)。结论肥胖儿童高UA血症与胰岛素抵抗、血脂代谢紊乱密切相关。Objective To explore the correlations between serum uric acid and insulin resistance as well as blood lipids in obese children. Methods 655 cases of obese children in Children’s hospital of Fuzhou/Fujian Medical University Teaching Hospital were selected as observation group,and other 655 cases of healthy non-obese children during the same period as control group,then the serum uric acid（UA）,fasting plasma glucose（FPG）,fasting insulin（FINS）,total cholesterol（TC）,triglyceride（TG）,high density lipoprotein cholesterol（HDL-C） and low density lipoprotein cholesterol（LDL-C） were measured. Furthermore,the insulin resistance index（HOMA-IR） was calculated,and observation group were divided into hyperuricemia group（HUA group） and normal UA group（NUA group） according to the serum UA level. Results The serum UA,FPG,FINS,TC,TG,LDL-C and HOMA-IR in observation group were significantly higher than those in control group（P〈0.05）,while the serum HDL-C was significantly lower than that in control group（P〈0.05）. The serum FPG,FINS,TG,and HOMA-IR in HUA group were significantly higher than those in the NUA group（P 〈0.05）,while no differences were found in serum TC,HDL-C and LDL-C between the two groups（P〉0.05）. By Pearson correlation analysis,the serum UA was positively correlated with FINS,TC,TG,LDL-C and HOMA-IR in obese children（P〈0.05）,while it was negatively correlated with HDL-C（P〈0.05）. Conclusion In obese children,there is a positive relationship between hyperuricemia and insulin resistance as well as lipid metabolism disorder.
