摘 要:目的 了解早产儿病房环境光线暴露情况,为后续研究光环境干预对早产儿昼夜节律影响提供基础。方法 在新生儿科早产儿病房,采用光度计,取房间对角线连线中点作为监测点,监测环境与暖箱内光线强度。结果日班环境的平均光线强度为(181.6±11.6)lux,暖箱内为(37.1±7.3)lux。夜班环境的平均光线强度为(86.8±5.6)lux,暖箱内为(36.8±10.8)lux。暖箱内光线暴露强度显著低于环境(均P〈0.01)。暴露环境、暴露时间及病房患儿数是影响光线暴露强度的主要因素(P〈0.05,P〈0.01)。结论 早产儿病房的日间光线总体暴露均值稍低,夜间光线稍强。若需要模拟昼夜节律的交替光照并达到所需的光照强度,日间需在窗帘完全遮光的情况下、通过控制日光灯开启的数量以达到200~600lux的目标光线暴露,而夜间则需应用眼罩,以达到〈20lux的目标光线强度暴露。Objective To investigate light exposure intensity in the intensive care unit for preterm infants and to provide information for luther study on the impact of environmental light on circadian rhythms in preterm infants. Methods The midpoint of the diagonal line of the preterm ward-room was used to display environmental light exposure intensity and the incubator was also monitored by using TES-1339R photometer. Results Light exposure intensity of the incubator, averaging (37. 1±7. 3)lux during day shift and (36.8 ± 10.8)lux in night shift, was significantly lower than that of the environment, averaging (181.6 ± 11.6)lux during day shift and (86.8±5.6)lux in night shift (P〈0.01 for both). The main predictors of light exposure intensity were exposure environ- ment, duration and the number of infants lived in (P〈0.05, P〈0.01). Conclusion Light exposure intensity in the intensive care unit for preterm infants was somewhat low during day shift and slightly high at night. In order to simulate circadian light and achieve required light intensity with the use of full shading curtains, the number of lightings should be controlled during day shift to achieve a target light intensity of 200--600 lux and eye mask should be provided at night to control the light intensity less than 20 lux.
