2011～2015年泽州县疑似肺结核患者转诊追踪情况分析

摘　　要：目的分析泽州县2011～2015年非结核病防治机构（简称“非结防机构”）网络直报肺结核或疑似肺结核患者到结核病防治机构（简称“结防机构”）就诊及诊断情况，提高患者发现率。方法利用结核病专报系统中2011～2015年泽州县非结防机构报告的2243例肺结核及疑似肺结核患者转诊、追踪及核查报表信息，对实际报告与转诊的1383例患者的转诊到位率、追踪到位率、总体到位率、到位后肺结核患者检出率及非结防机构对肺结核患者发现的贡献率进行分析。结果2011～2015年泽州县非结防机构网络直报肺结核及疑似肺结核患者转诊到位率分别为64．49％（109／169）、68．25％（172／252）、70．46％（198／281）、81．27％（204／251）、91．63％（394／430），呈逐年上升趋势；追踪到位率分别为81．67％（49／60）、87．50％（70／80）、95．18％（79／83）、97．87％（46／47）、100％（36／36）；总体到位率分别为94．08％（159／169）、96．03％（242／252）、98．57％（277／281）、99．60％（250／251）、100％（430／430）；到位活动性肺结核诊断符合率为45．80％（622／1358），涂阳肺结核的检出率24．08％（327／1358）；非结防机构对活动性肺结核患者发现的贡献率为57．01％（622／1091）；对涂阳肺结核患者发现的贡献率57．36％（327／570）。结论泽州县2011～2015年非结防机构网络直报肺结核患者的转诊到位率呈逐年上升趋势，追踪到位率及总体到位率维持在较高水平。Objective To understand the status of referral and arrival and diagnosis of pulmonary tuberculosis （PTB） patients or suspicious TB patients reported by internet-based reporting system by non-TB control institutions in Zezhou county,2011 2015,so as to improve the case detection rate. Methods The referral, tracing and checking report from 2 243 PTP patients reported by non-TB control institutions in tuSerculosis management information system of Zezhou county, 2011-2015 was used. The referral arrival rate, tracing arrival rate, total arrival rate, detection rate and the contribution rate on TB case detection by non-TB institutions of 1 383 cases referred was analyzed. Results The referral arrival rate among the patients reported in internet-based surveillance system by non-TB control institutions during 2011-2015 was 64.49% （109/169）,68.25% （172/252）,70.46% （198/281）,81.27% （204/251）,and 91.63%（394/430）respectively,with a rising trend year by year. The tracing arrival rate was 81.67% （49/60） ,87.50% （70/80）,95.18~/00 （79/83）, 97.87% （46/47） ,and 100%（36/36）. The total arrival rate was 94.08% （159/169） ,96.03% （242/252） ,98.57% （277/ 281）, 99.60 % （250/251 ）, and 100 % （430/430） respectively. Among arrival patients, the detection rate of active PTB was 45.80% （622/1 358） and that of smear positive PTB was 24.08% （327/1 358）. The contribution rate on active PTB case detection by non-TB institutions was 57.01% （622/1 091） and that of smear positive PTB was 57.36% （327/570）. Conclusion The referral arrival rate is increasing year by year in Zezhou county, 2011-2015 , and the tracing arrival rate and total arrival rate maintain at a high level.
