46例围生期耻骨联合分离的临床研究

摘　　要：目的 探讨妊娠合并耻骨联合分离的病因。方法 选取2011年1月-2014年1月该院收治的因妊娠合并耻骨联合分离住院分娩且随访至产后6个月的患者46例为研究对象,并对其临床资料进行总结分析。结果 妊娠合并耻骨联合分离与胎儿过大、胎头高浮等有关,采取一系列的有效治疗后产妇自觉疼痛明显减轻。46例患者均痊愈。产妇在治疗当天视觉模拟评分（VAS）大幅度下降,与治疗前比较,差异有统计学意义（P〈0.01）;治疗20~30 d后VAS评分≤2分,已基本达到治愈标准;观察组的手术助产率、新生儿体重和胎头高浮发生率明显高于对照组,差异均有统计学意义（P〈0.05）。结论 围生期孕产妇耻骨联合分离由胎儿过大、胎头高浮等多种原因造成;根据临床表现及B超检查可以早期作出诊断;确诊后及时做好不同时段的处理,可明显降低孕产妇痛苦,提高生活质量、促进康复。Objective To explore the causes of symphyseolysis during pregnancy. Methods Forty-six patients with symphyseolysis during pregnancy treated in the hospital from January 2011 to January 2014 were selected, all the patients were followed up for six months af- ter delivery, the clinical data was summarized and analyzed. Results Symphyseolysis during pregnancy was correlated with fetal oversize and high position of fetal head. After taking a series of effective treatments, the maternal perceived pain significantly relieved. All the 46 pa- tients were cured. YAS score of the pregnant women on the day of treatment dropped significantly, compared with before treatment, the difference was statistically significant （P〈0. 01 ） ; after 20-30 days of treatment, VAS score was 2 points or less than 2 points, which basi- cally reached the cure standard; midwifery rate, neonatal weight, and incidence rate of high position of fetal head in observation group were statistically significantly higher than those in control group （ P〈0.05 ） . Conclusion Symphyseolysis during pregnancy is caused by multiple reasons, such as fetal oversize and high position of fetal head; according to the clinical manifestations and B ultrasound examination, symphyseolysis can be diagnosed early; after definite diagnosis, timely treatment during different time periods can significantly reduce materhal pain, improve the quality of life, and promote rehabilitation.
