分娩镇痛对产妇产程及新生儿的影响研究

摘　　要：目的分析分娩镇痛对产妇产程及新生儿的影响。方法选择317例初产妇随机分为两组,分为分娩镇痛组和不施行镇痛组(对照组),比较两组分娩疼痛、产程、产后并发症、新生儿Apgar和NBNA评分。结果当宫口开2 cm时,两组之间的疼痛评分差异不具有统计学意义(P>0.05),但当宫口开至4 cm、6 cm、8 cm乃至10 cm时,两组之间的疼痛评分差异均具有统计学意义(P<0.05),宫口从2 cm开至10 cm,两组产妇所需时间的差异具有统计学意义(P<0.05),但第二产程和第三产程所需时间的差异却无统计学意义(P>0.05)。两组产妇的分娩方式及产后并发症、新生儿Apgar和NBNA评分比较,两组之间的差异都不具有统计学意义(P>0.05)。结论采用镇痛分娩虽然可能会使产程时间有所延长,但镇痛分娩对产妇的分娩方式、产后并发症的发生率以及新生儿的健康都无显著影响,而且镇痛分娩还能降低产妇分娩时的疼痛,值得在临床上广泛推广。Objective To analyze the effect of labor analgesia on labor process and neonates. Methods A total of 317 primiparous women were selected and randomly divided into labor analgesia group and control group （without labor analgesia） . The pain scores, labor stages, postpartum complications, neonatal Apgar scores and NBNA scores in the two groups were compared. Results When orifice of the . uterus opened to 2 cm, there was no statlshcally significant difference between the two groups （P〉0. 05 ） ; when orifice of the uterus opened to 4, 6, 8, and 10 cm, there were statistically significant differences between the two groups （P〈0. 05 ） ; there was statistically significant difference in the interval time of orifice of the uterus opening from 2 cm to 10 cm （P〈0. 05 ） ; there was no statistically significant difference in the time of the second stage and the third stage of labor between the two groups （P〉0. 05 ） . There was no statistically significant differ- ence in delivery modes, postpartum complications, neonatal Apgar and NBNA scores between the two groups （P 〉 0. 05 ） . Conclusion Although labor analgesia may prolong the time of stages of labor, labor analgesia has no significant impact on delivery modes, the incidence rates of postpartum complications, and neonatal health, labor analgesia also can reduce labor pain, which is worthy of clinical application.
