摘　　要：目的探讨脓毒症患者早期检测血小板及其相关参数的临床意义。方法选择2014年1月—2015年12月解放军白求恩国际和平医院急诊科收治的符合纳入及排除标准的脓毒症78例,根据入院当日血小板计数,分为血小板减少组（血小板〈100×109/L,32例）和血小板正常组（血小板≥100×10^9/L,46例）,分析比较两组一般资料,并采用Pearson相关分析对血小板计数与血小板相关参数的关系进行分析。结果与血小板正常组比较,血小板减少组血小板平均容积（MPV）、血小板分布宽度（PDW）、大血小板比例（P-LCR）、感染性休克发生率及病死率均升高,且重症监护室住院时间延长,差异均具有统计学意义（P〈0.05）。血小板减少组腹部感染率高于血小板正常组,肺部感染率低于血小板正常组,差异均有统计学意义（P〈0.05）。Pearson相关分析显示血小板计数与MPV、PDW及P-LCR均呈负相关。结论脓毒症患者早期检测血小板及其相关参数可判断病情严重程度并对其预后进行评估,能为临床进一步治疗提供参考依据。Objective To investigate clinical significance of early detection of platelet and related parameters for patients with sepsis. Methods A total of 78 patients with sepsis during January 2014 and December 2015,who fitted internalizing and evacuating standard were divided into thrombocytopenia group（ platelet 100 × 109/ L,n = 32） and normal group（ platelet≥100 × 10^9/ L,n = 46） according to platelet count at the time of admission. In two groups,general data was analyzed,and relationship between platelet count and its related parameters was also analyzed using Pearson correlation analysis.Results Values of mean platelet volume（ MPV）,platelet distribution width（ PDW）,platelet large cell ratio（ P-LCR）,incidence and mortality rates of septic shock were increased,and length stay in ICU was lengthened in thrombocytopenia group compared with those in normal group（ P〈0. 05）. Abdominal infection rate was higher,while pulmonary infection rate was lower in thrombocytopenia group than those in normal group（ P〈0. 05）. Pearson correlation analysis showed that platelet count were negatively correlated with MPV,PDW and P-LCR. Conclusion Early detection of platelet and related parameters for patients with sepsis can evaluate severity of the disease and prognosis,and therefore it may provide reference for further clinical treatment.
