曲安奈德联合盐酸氨溴索治疗分泌性中耳炎患者的临床疗效及对机体炎性因子、细胞免疫指标的影响

摘　　要：目的:探究曲安奈德联合盐酸氨溴索治疗分泌性中耳炎患者的临床疗效及对机体炎性因子、细胞免疫指标的影响。方法:随机筛选本院门诊40例单耳分泌性中耳炎患者作为试验组,采用曲安奈德联合盐酸氨溴索治疗方式;随机筛选同期40例单耳分泌性中耳炎患者作为对照组,采用地塞米松治疗方式。观察两组患者的临床疗效,检测两组患者治疗前后的炎性因子和细胞免疫指标水平。结果:试验组总有效率(90%)显著高于对照组总有效率(80%)(P<0.01)。治疗前两组患者的血清炎性因子(IL-5、IL-8、IL-1β、PCT、TNF-α)、耳积液炎性因子(IL-5、IL-8、IL-1β、PCT、TNF-α)以及T淋巴细胞亚群(CD4^+、CD8^+、CD4^+/CD8^+)水平比较差异均无统计学意义(P>0.05)。治疗后,两组患者的血清及耳积液炎性因子水平与本组治疗前相比均显著性降低(P<0.05或P<0.01),两组患者的CD4^+、CD4^+/CD8^+水平与本组治疗前相比均显著性升高(P<0.05),而CD8^+水平与本组治疗前相比显著性降低(P<0.01)。治疗后,试验组的血清及耳积液的上述炎性因子水平分别明显低于对照组(P<0.05或P<0.01),但两组的血清及耳积液IL-5水平比较无显著性差异(P>0.05);试验组的CD4^+/CD8^+水平显著高于对照组(P<0.05),但两组的CD4^+、CD8^+水平比较无显著性差异(P>0.05)。结论:曲安奈德联合盐酸氨溴索的治疗方式更有助于改善分泌性中耳炎患者的免疫功能、改善治疗效果,对分泌性中耳炎患者的治疗具有十分重要的临床意义。Objective：To explore the clinical effects of triamcinolone acetonide ammonia bromine joint hydrochloric acid on patients with secretory otitis media and the influences on the body＇s inflammatory factors and cellular immunologic parameters.Methods：A total of 40 cases of single ear secretory otitis media patients were randomly selected as experimental group,and were given triamcinolone acetonide ammonia bromine joint hydrochloric acid treatment;And 40 cases of single ear secretory otitis media patients of the same period were randomly selected as control group,and were given dexamethasone treatment.To observe the clinical efficacy and detect the inflammatory factors as well as cellular immune index level of the two groups before and after treatment.Results：The total effective rate（90%）of treatment group is significantly higher than that（80%）of control group（P〈0.01）.The comparative of the serum inflammatory factors（IL-5、IL-8、IL-1β、PCT、TNF-α）,ear effusion inflammatory cytokines（IL-5、IL-8、IL-1＋β、PCT、TNF-α）and T lymphocyte subsets（CD4^＋、CD8^＋、CD4/CD8^＋）levels of the two groups before treatment were not statistically significant（P〈0.05）.After treatment,the serum and ear effusion inflammatory cytokines levels of the two groups were significantly lower than that before treatment（P 〈0.05 or P〈 0.01）,the CD4^＋and CD4^＋/CD8^＋levels of two groups of patients were significantly higher than that of the same group before treatment（P〈 0.05）,while the CD8 ＋levels were significantly lower than the same group before treatment（P〈 0.01）.After treatment,the serum and ear effusion inflammatory factor levels of the experimental group were significantly lower than that of the control group（P 〈0.05 or P 〈0.01）respectively,but the two groups of serum and ear effusion IL-5levels were no significant difference（P〈0.05）;The CD4＋/CD8＋levels of experimental group were significantly higher than the control group（P 0.05）,but dif
