摘 要:目的探讨血清内脏脂肪组织源性丝氨酸蛋白酶抑制剂(Vaspin)、趋化素(Chemerin)的水平与妊娠期糖尿病(GDM)患者肥胖及胰岛素抵抗的关系。方法根据GDM、肥胖诊断标准将309例孕妇分为肥胖GDM组、BMI正常GDM组、健康对照组各103例。检测各组血清Vaspin、Chemerin水平及空腹血糖(FPG)、空腹胰岛素(FINS),计算胰岛素抵抗指数(HOMA-IR);用Pearson相关分析Vaspin、Chemerin与FPG、FINS、HOMA-IR的关系,用多元逐步回归分析多个自变量与1个应变量的关系。结果与健康对照组比较,GDM组FPG、FINS、HOMA-IR水平升高(P均<0.05);肥胖GDM组FINS、HOMA-IR高于BMI正常GDM组(P均<0.05)。Pearson相关分析结果显示,Vaspin与BMI、FPG、FINS、HOMA-IR呈正相关(r分别为0.340、0.318、0.290、0.307,P均<0.05);Chemerin与BMI、FPG、FINS、HOMA-IR无相关性(r分别为0.117、0.141、0.204、0.219,P均>0.05)。多元逐步回归分析结果显示,BMI、FPG、FINS是GDM患者血清Vaspin水平的影响因素(B'分别为0.414、0.392、0.487,P均<0.05),BMI、FINS是GDM患者血清Chemerin水平的影响因素(B'分别为0.255、0.157,P均<0.05)。结论 GDM患者血清Vaspin、Chemerin水平升高,二者均与肥胖及IR有关。Objective To investigate the relationships of visceral adipose tissue derived serine protease inhibitor( Vaspin) and Chemerin with obesity and insulin resistance( IR) in patients with gestational diabetes mellitus( GDM).Methods According to the diagnostic standard of obesity and GDM,we divided the pregnant women into the obesity GDM group of progestation,normal GDM group of progestation and healthy control group,103 cases in each group,respectively.Testing the levels of fasting plasma glucose( FPG),fasting insulin( FINS),Vaspin and Chemerin in the serum of each group and calculating insulin resistance index( HOMA-IR). The relationships between Vaspin,Chemerin and FINS,HOMA-IR,FPG were analyzed by Pearson correlation,and the relationships between multiple independent variables and one dependent variable were analyzed by multiple stepwise regression. Results Compared with the healthy control group,the levels of FINS,FPG and HOMA-IR were elevated in the GDM group( all P〈0. 05). The FINS and HOMA-IR in the obesity GDM group of progestation were higher than those in the normal GDM group of progestation( all P〈0. 05). The Pearson correlation analysis showed that Vaspin was positively correlated with BMI,FPG,FINS,HOMA-IR( r = 0. 340,0. 318,0. 290 and0. 307,all P〈0. 05). Chemerin had no correlation with BMI,FPG,FINS,HOMA-IR( r = 0. 117,0. 141,0. 204 and0. 219,all P〉0. 05). The multiple stepwise regression analysis showed that BMI,FPG and FINS were the influencing factors for Vaspin level in serum of GDM patients( B' = 0. 414,0. 392 and 0. 487,all P〈0. 05),FINS and BMI were the influencing factors for Chemerin level in serum of GDM patients( B' = 0. 255 and 0. 157,all P〈0. 05). Conclusion The levels of Chemerin and Vaspin increase in serum of GDM patients and both of them are related to obesity and IR.
