摘　　要：目的探索社区泌尿系感染产ESBLs大肠埃希菌对医院感染防控的警示因素。方法用前瞻性和回顾性相结合的方法,调查入住福建医科大学附属第二医院的社区泌尿系感染大肠埃希菌的病例,采用多因素logistic回归分析警示产ESBLs大肠埃希菌的因素。结果共纳入患者106例,产ESBLs组68例,非产ESBLs组38例。对两组的7个方面共45个因素进行多因素logistic回归分析。患者有留置尿管史、在入院前3个月内使用抗生素、血常规白细胞≥15.0×109/L、贫血和使用三代头孢菌素和泌尿系统手术史为泌尿系感染产ESBLs大肠埃希菌的警示因素。结论应合理使用抗生素,减少尿管留置,纠正贫血,以减少社区产ESBLs大肠埃希菌尿路感染。对有警示因素的患者,应及时采取干预措施,防止医院内传播和扩散。Objective To explore the warning factors for community-acquired ESBLs urinary tract infection for the disease control and prevention.Methods Combined with prospective and retrospective studies,the hospitalized cases in The Second Affiliated Hospital with community-acquired ESBLs urinary tract infection were investigated and the warning factors were analyzed by multiple factor logistic regression model.Results Totally 106 cases were divided into ESBLs group（68cases）and non-ESBLs group（38cases）.Totally 45 factors in 7aspects were analyzed by the model.History of indwelling catheter,antibiotics therapy within 3months prior to hospitalization,WBC count≥15.0×10^9/L,anemia,third generation cephalosporin therapy and history of urinary system operations were the warning factors.Conclusion Antibiotics therapy reasonably,minimizing indwelling catheter and anemia treatment may reduce the community-acquired ESBLs urinary tract infection.Comprehensive intervention measure should be taken to the patients with the warning factors for preventing the disease spread and proliferation in hospital.
