Th细胞相关细胞因子水平与老年湿疹患者临床特征的相关分析

摘　　要：目的研究老年湿疹患者血清辅助性T细胞因子的水平及其与湿疹临床特征的相关性。方法收集2013年9月-2015年9月在我科确诊为老年湿疹患者60例的临床资料,并选取健康对照组40例。采用酶联免疫吸附法(ELISA)检测血清中Th的特征细胞因子γ-干扰素(INF-γ)、白介素-4(IL-4)及白介素-17A(IL-17A)的表达水平,分别比较Th细胞相关因子在急慢性,或泛发和局限型湿疹中的差异。结果与老年健康对照组相比,老年湿疹患者血清IFN-γ,IL-4,IL-17A显著升高(P均<0.001)。其中老年急性湿疹患者血清IFN-γ,IL-4及IL-17A显著高于慢性期(P均<0.001);泛发型患者血清IFN-γ,IL-4及IL-17A明显高于局限型(P均<0.05)。结论血清IFN-γ,IL-4及IL-17A可能参与老年湿疹的进展与预后,并且Th细胞相关因子表达的不同可能作为反映临床类型的指标。Objective To investigate the serum levels of helper T lymphocyte cytokines and its association with clinical features in the elderly with eczema. Methods The clinical data of 60 cases of the elderly eczema patients were collected from September 2013 - September 2015 in our department. 40 cases of healthy subjects served as normal controls. Enzyme-linked immuno sorbent assay（ELISA） was used to measure the levels of helper T lymphocyte cytokines, including INF-γ, IL-4 and IL-17A in both normal controls and patients with eczema. The cytokine levels were compared between acute and chronic, generalized and localized eczema. Results The levels of IFN-γ, IL-4 and IL-17A in the elderly patients with eczema were higher than that in normal controls （P 〈 0. 001 ） ; The serum cytokine levels in acute eczema were higher than that in chronic eczema （ P 〈 0. 001 ） ; All three cytokine levels in generalized eczema were higher than that in localized eczema （P 〈 0.05 ）. Conclusion INF-γ, IL-4 and IL-17A may play the important role in the progress and prognosis of eczema in the elderly. The levels of INF- γ, IL-4 and IL-17A could be used as a marker of eczema status.
