摘 要:目的:从乡村医生的视角对北京市H区乡村医生管理存在的问题进行调查,探讨产生问题的原因。方法:采取目的抽样的方法在北京市H区选取3个乡镇的18个行政村作为现场,采用参与式与非参与式观察相结合的方法,对乡村医生进行实地观察和个人深入访谈。结果:北京市H区在乡村医生管理上存在管理主体不明、激励机制匮乏、公共卫生与基本医疗服务结构不合理、乡村医生待遇及社会保障欠缺等问题。结论:我国政权制度的转变以及政策制定的思维惯性是导致乡村医生管理主体不清的主要原因。激励手段的缺乏会降低乡村医生工作积极性,乡村两级医疗重点不明确制约了农村三级医疗网络功能的发挥,而乡村医生保障制度不健全导致乡村医生人才匮乏,农村三级医疗卫生网络功能难以发挥。Objective:We investigated the existing management problems from the perspective of rural doctors in H District of Beijing to ex- plore reasons of these issues. Methods: we selected 18 administrative villages in three towns in H District of Beijing by taking purposive sampling method. The field observation and in-depth interview were conducted with rural doctors by using method combining participatory with non-partici- patory observation. Results :There were problems in rural doctor management in H District of Beijing such as unclear management subjects, lack of incentive mechanism, unreasonable public health and basic medical service structure and inadequate protection of treatment. Conclusion: The change of regime system in our country and the inertia of thinking in policy making were the main reasons for unclear management subjects of rural doctors.Lack of incentivemeans would reduce the enthusiasm of rural doctors.The unclear medical treatment keys of township and village restricted the function of the three level medical network in rural areas. The unsound security system would lead to talent dilemma of rural doctors.
