摘 要:目的 观察电针对切口痛大鼠的镇痛效应及背根神经节瞬时受体电位香草酸亚型1(TRPV1)及其相关信号通路下游效应分子表达的影响,探讨电针镇痛的神经机制.方法 30只雄性Sprague-Dawley大鼠随机分为3组:空白对照组(C组)、切口痛组(Ⅰ组)、电针组(EA组).除C组外,其余两组行切口痛模型造模.各组大鼠于术前1h、术后6、24、48 h,测定机械缩足反射阈(MWT)值.Western blot法检测背根神经节TRPV1和电压门控性钠离子通道1.7(Nav1.7)和电压门控性钠离子通道1.8(Nav1.8)的表达变化.结果 与术前比较,Ⅰ组行切口痛模型造模后术后各时间点的MWT阈值均明显降低[术前和术后6、24、48 h的MWT值分别为(23.71±2.02)、(4.59±1.15)、(7.37±1.36)、(9.58±1.32)g,与同组术前基础值比较,差异有统计学意义(P<0.05)];EA组行切口痛模型造模后与术前基础值比较,MWT阈值也均明显降低(P<0.05)[术前和术后6、24、48 h的MWT值分别为(23.58±1.96)、(6.17±1.91)、(10.88±1.79)、(12.27±1.68)g].但与Ⅰ组比较,经EA处理后,EA组术后6、24、48 h的MWT阈值明显升高(P<0.05).同样,与C组比较,Ⅰ组和EA组大鼠术后48 h脊髓背根神经节TRPV1、Nav1.7和Nav1.8表达增加(P<0.05);但EA处理后,脊髓背根神经节TRPV1、Nav1.7和Nav1.8表达水平下调,与Ⅰ组比较,差异有统计学意义(P<0.05).结论 电针可抑制大鼠切口痛模型所致的术后疼痛,可能与抑制TRPV1及其相关信号通路有关.Objective To investigate the effects of electroacupucnture (EA) on the postoperative pain and the expression of transient receptor potential vanilloid 1 (TRPV1) and related downstream effectors of dorsal root ganglion (DRG) in the rat model of incisional pain.Methods Thirty adult male Sprague-Dawley rats were randomly divided into 3 groups (n =10 each):control group (C group),incision group (Ⅰ group),and EA group.In the Ⅰ group and EA group,the model of incisional pain on the right plantar surface was established.At 1 st h before modelling,and 6,24 and 48 h after modelling respectively,withdrawal mechanical threshold (WMT) was detected in right hind paw.Western blotting was used to detect the expression of TRPV1 and sodium channel-associated proteins (Nav1.7 and Nav1.8).Results As compared with the preoperative baseline,Ⅰ group displayed a profound decrease in WMT at each time point after model establishment [(23.71 ±2.02),(4.59 ± 1.15),(7.37 ± 1.36),and (9.58 ± 1.32) g at preoperative baseline,postoperative 6,24,and 48 h,respectively;P 〈 0.05].As compared with the preoperative baseline,EA group also displayed a decrease in WMT at each time point after model establishment [(23.58 ± 1.96),(6.17 ± 1.91),(10.88 ± 1.79) and (12.27 ± 1.68) g at preoperative baseline,postoperative 6,24,and 48 h,respectively,P 〈 0.05].However,as compared with Ⅰ group,the WMT at 6,24 and 48 h after modeling was significantly increased in EA group (P 〈 0.05).Meanwhile,as compared with C group,the levels of TRPV1,Nav1.7 and Nav1.8 at 48 h after modeling were significantly increased in DRG in Ⅰ group and EA group (P 〈 0.05).As compared with Ⅰ group,the levels of TRPV1,Nav1.7 and Nav1.8 at 48 h after modeling were down-regulated in DRG in EA group (P 〈 0.05).Conclusion EA could produce analgesic effect on post-operation pain in the rat incision pain model,which is associated with inhibition of TRPV1 and associated signaling pathway.
