摘  要:目的 模仿经椎弓根置钉引起的急性腰骶部神经根损伤,探讨下肢胫后神经F波连续监测对发现兔急性神经根损伤敏感性及其对神经功能的预测价值.方法 12只新西兰大白兔随机分为3组,对照组(n =4)用于排除麻醉和手术对F波的影响,实验组(n=4)随机分为1个血管夹组和2个血管夹组,应用血管夹夹压坐骨神经后连续记录F波2h,分析F波的波幅变化与坐骨神经夹压后2d后下肢运动功能的相关性.结果 对照组麻醉和手术后不同时间F波的波幅和潜伏期经方差分析差异无统计学意义(P>0.05);实验组血管夹压损伤前后的F波波幅与基线波幅百分比分别稳定在(74.6±8.6)%、(2.7±3.7)%;潜伏期分别稳定在(11.6±1.0)、(12.6±0.3) ms,波幅与潜伏期经t检验比较差异均有统计学意义(P<0.05);F波的波幅变化与坐骨神经夹压2d后运动功能呈正相关(r =0.871,P<0.01).结论 F波对发现急性神经根损伤敏感,可以在神经根损伤的急性期(5 min内)发现神经根损伤,从而可以采取保护性措施预防不可逆性神经根损伤的发生.Objective Explore the sensitivity of F wave continuous monitoring of posterior tibial nerve to the acute nerve root injury in rabbits with acute lumbar and sacral nerve root injury caused by pedicle screw insertion.Methods 12 New Zealand white rabbits were randomly divided into 3 groups,control group (n =4) used to exclude the influence of anesthesia and surgery on F wave,experimental group (n =4) were randomly divided into one clip group and two clips group,continuous record F wave 2 h after pressure the sciatic nerve,analyze the correlation between the amplitude of F wave and the motor function of lower limb after 2 days.Results After anesthesia and surgery the F wave amplitude and latency variance difference analysis was not statistically significant (P 〉 0.05);The F wave amplitude and baseline amplitude percentages of experimental group were stable at (74.6 ±8.6)%,(2.7 ±3.7)%;incubation period were stable in (11.6 ± 1.0),(12.6 ± 0.3) ms.Amplitude and latency by T-test differences were statistically significant (P 〈 0.05);F wave amplitudes and the motor function of lower limb was positively correlated (r =0.871,P 〈 0.01).Conclusion F wave was found to be sensitive to acute nerve root injury,which could be found in the acute phase (5 min) of nerve root injury,which could be taken to prevent the occurrence of irreversible nerve root injury.
