摘 要:目的分析维持性血液透析患者导管相关血流感染(CRBSI)的病原菌分布特点及耐药性。方法回顾性分析36例维持性血液透析CRBSI患者血培养结果,统计病原菌的分布情况及其对常见抗菌药物的耐药性。结果 36例患者中血培养分离菌株共30株,其中革兰阳性菌12株,以表皮葡萄球菌为主;革兰阴性菌18株,以铜绿假单胞菌及嗜水气单胞菌为主。分离的细菌对临床常见抗菌药物呈现多重耐药。结论维持性血液透析患者CRBSI以革兰阴性菌为主,应当根据药敏试验结果合理选择抗菌药物。Objective To investigate the pathogen distribution and drug resistance of catheter related bloodstream infections(CRBSI) in the patients with maintenance hemodialysis. Methods The results of blood culture in 36 CRBSI patients with maintenance hemodialysis were retrospectively analyzed. The distribution of pathogens and their drug resistance to common antibiotics were analyzed. Results Of 36 cases with maintenance hemodialysis, 30 strains of pathogens were cultured and isolated. Gram-positive bacteria was found in 12 strains, of which Staphylococcus epidermis was the main bacteria. Gram-negative bacteria was seen in 18 strains, which mainly composed by Pseudomonas aeruginosa and Aeromonas hydrophila. The isolated bacteria were resistant to multiple clinical antibiotics. Conclusion CRBSI in the patients with maintenance hemodialysis is mainly caused by Gram-negative bacteria. Antibiotics should be reasonably selected based on the result of drug sensitivity test.
