摘 要:目的:观察加味四物汤对阴虚阳亢型高血压患者的治疗效果。方法:阴虚阳亢型高血压患者100例,分为观察组和对照组,对照组给予氨氯地平片口服控制血压,观察组在对照组治疗的基础上加用加味四物汤联合治疗,记录两组患者于治疗前和治疗后8周时的收缩压、舒张压、左心射血分数(LVEF)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVDs),同时检测两组患者血清血管内皮素-1(ET-1)、一氧化氮(NO)、总三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)水平,统计患者肝功或肾功异常患者数并计算异常率。结果:治疗前,两组患者收缩压、舒张压、LVEF、LVEDD、LVDs及血清ET-1、NO、LDL、TC、HDL、TG水平分别比较,差异无统计学意义(P>0.05);治疗后,两组患者收缩压、舒张压、LVEDD、LVDs及血清ET-1、LDL、TC及TG水平明显降低,LVEF及血清NO、HDL水平升高,差异有统计学意义(P<0.05);与对照组比较,观察组变化更明显,差异有统计学意义(P<0.05);治疗后,观察组1例发生肝功指标异常,对照组8例发生肝或肾功能异常,两组异常率比较,差异有统计学意义(P<0.05)。结论:加味四物汤降压效果优于氨氯地平,其降压机制可能与ET-1降低和NO升高有关。Objective:To observe the curative effect of jiaweisiwutang on patients with hypertension of yin deficiency yang excess type. Methods:100 patients with hypertension of yin deficiency yang ex-cess type were divided into observation group and control group. The control group was given amlodip-ine tablets to control blood pressure,based on the treatment in the control group,observation group ac-cepted jiaweisiwutang. Systolic pressure and diastolic pressure, left ventricular ejection fraction ( LVEF),left ventricular end diastolic diameter( LVEDD)and left ventricular end systolic diameter ( LVDs)of the patients in the two groups were recorded before treatment and 8 weeks after treatment. Serum levels of endothelin-1(ET-1),nitric oxide(NO),total triglyceride(TG),total cholesterol ( TC),low density lipoprotein cholesterol( LDL-C)and high density lipoprotein cholesterol( HDL-C) were detected,the abnormal incidence of liver function and renal function in the two groups were sta-tistically analyzed. Results:Before treatment,the differences in systolic blood pressure,diastolic blood pressure,LVEF,LVEDD,LVDs and serum levels of ET-1,NO,LDL,TC,HDL,TG of the two groups were not statistically significant(P〉0. 05);After treatment,systolic blood pressure and diastolic blood pressure,LVEDD,LDL,LVDs and serum levels of ET-1,TC and TG in the two groups were significantly decreased,LVEF and serum levels of NO,HDL in the two groups were increased, the differences were statistically significant(P〈0. 05). Compared with the control group,the changes in observation group were more obvious,the difference was statistically significant(P〈0. 05). In the observation group,there were 1 cases of patients with liver( renal)function abnormalities,8 cases in the control group,the difference was statistically significant(P〈0. 05). Conclusion:Antihypertens-ive effect of jiaweisiwutang is better than amlodipine,its antihypertensive mechanism may be related to the decrease of ET-1
