七厘散穴位贴敷对髋膝关节置换术后深静脉血栓形成的防治

摘　　要：目的:探讨中医治疗对髋膝关节置换后深静脉血栓(DVT)形成的防治效果。方法:将80例本病患者随机分成两组。实验组(40例)采用穴位贴敷联合基础治疗;对照组(40例)单纯基础治疗。比较两组在防治髋膝关节置换术后深静脉血栓形成的治疗效果和安全性。结果:对照组术后发生DVT 9例,实验组发生2例,两组DVT发生率有差别。两组患者凝血四项及D-dimer用药前无差别。两组患者凝血四项中PT、FIB、APTT用药后无差别;实验组TT高于对照组及D二聚体低于对照组。两组病例均未发生严重出血及大伤口血肿,伤口引流量实验组较对照组低。结论:基础治疗加七厘散穴位贴敷防治髋膝关节置换术后深静脉血栓形成的效果较好。Objective：To investigate the efficacy of Chinese traditional treatment on the prevention of deep venousthrombosis（DVT）aftertotalhipandkneereplacement .Methods：Thecases（80）relatedtotheeffectofChinesetra-ditional treatment on the prevention of DVT after total hip and knee replacement were collected to analyze compare the efficacy and safety of experimental group （40cases）：acupoint topical application of drug with basic treatment and control group（40cases）：basic treatment alone on the treatment and prevention of DVT after total hip and knee replacement .Re-sults ：In the whole experiment ,there were 11 cases having DVT （13 .75% ） ,in the control group there were 9 cases having DVT ,in the experimental group there was 2 case having DVT （P〈0 .05） .Before the treatment ,the value of each index in tow groups had no statistical significance （P〉0 .05） .After the treatment for 7 days ,the value of PT 、FIB＆amp;APTT in tow groups had no statistical significance （P〉0 .05） .The value of TT in experimental group was higher than in control group ,and the value of Ddimer was lower than that in control group （P〈0 .05） .There was no severe bleeding case and large hematoma case in tow groups .The blood drainage in experiment group was less than control group （P〈0 .05） . Conclusion ：On the prevention of DVT after total hip replacement and total knee prosthesis ,the efficacy of acupoint topical application of Qili san with basic treatment produce better result and high security ,which is worthy of clinical promo-tion .
