腹腔镜胆总管切开取石一期缝合不放置引流管治疗胆总管结石30例  

30 Cases of Common Bile Duct Stones Treated with Laparoscopic Common Bile Duct Incision without Placement of Drainage Tubes in Primary Suture

摘　　要：目的:观察腹腔镜胆总管切开取石一期缝合不放置引流管治疗胆总管结石的疗效。方法:选取医院在2014年6月至2016年6期间收治的胆总管结石患者60例为研究对象,并根据入院编号的不同分为观察组和对照组,观察组患者行腹腔镜下胆总管切开取石一期缝合不放置引流管治疗,而对照组则进行T管引流的放置治疗。结果:两组患者的手术均获得成功。观察组患者的术后住院时间和术中出血量与对照组相比无统计学差异(P>0.05);观察组患者手术时间、术后胃肠功能恢复时间、身体恢复时间以及住院费用明显优于对照组,差异有统计学意义(P<0.05);两组患者均出现不同程度的并发症情况,但观察组患者发生率(10.0%)低于对照组(26.7%),差异有统计学意义(P<0.05)。结论:腹腔镜胆总管切开取石一期缝合不放置引流管治疗胆总管结石明显优于放置引流管的治疗效果,缩短手术时间和胃肠功能恢复时间,降低术后的疼痛程度,减少并发症的发生。Objective:To analyze the curative effects of laparoscopic common bile duct incision and primary suture without drainage tubes in the treatment of common bile duct stones.Methods:60cases of patients with common bile duct stones in the People's Hospital of Pingxiang from June 2014 to June 2016 had been chosen and divided into the observation group and the control group according to the admission numbers,the observation group undergoing laparoscopic common bile duct incision and primary suture without drainage tubes,the control group undergoing T-tube drainage.The curative effects had been comparatively analyzed.Results:The two groups of patients were successfully operated.The observation group was compared with the control group in terms of postoperative bleeding and the hospitalization time,and the differences were not statistically significant(P>0.05);the operation time,the recovery time of postoperative gastrointestinal function,the recovery time in the observation group were shorter than the control group,with less hospital costs,the differences being statistically significant(P<0.05).The patients in the two groups suffered from complications to varying degrees with a lower incidence in the observation group,and the difference was statistically significant(P <0.05).Conclusion:Laparoscopic common bile duct incision and primary suture without drainage tubes in the treatment of common bile duct stones can achieve better effects than the T-tube drainage treatment,which not only can shorten the operation time,the recovery time of gastrointestinal function and reduce postoperative pain,but also can reduce the complications.
