疏肝清解汤治疗肝经郁热型青春期多囊卵巢综合征的远期疗效观察

摘　　要：目的观察疏肝清解汤治疗肝经郁热型青春期多囊卵巢综合征(PCOS)的远期临床疗效。方法将86例肝经郁热型青春期PCOS患者随机分为2组,对照组43例予炔雌醇环丙孕酮片治疗,治疗组43例予疏肝清解汤治疗,2组均治疗6个月经周期。比较2组停药6个月后体质量、多毛评分[改良Ferriman-Gallwey(m FG)评分系统]、卵巢体积、性激素水平[睾酮(T)、卵泡刺激素(FSH)、黄体生成激素(LH)],记录月经周期恢复情况。结果治疗组治疗总有效率95.3%,明显高于对照组(74.4%,P<0.05)。治疗组停药6个月后体质量、体质量指数(BMI)、m FG评分、卵巢体积、T、LH、LH/FSH均低于对照组(P<0.05),FSH与对照组比较差异无统计学意义(P>0.05)。治疗组月经周期恢复正常率(90.7%)高于对照组(55.8%,P<0.05)。结论疏肝清解汤治疗肝经郁热型青春期PCOS的临床效果显著,能有效改善患者临床症状及激素水平,其作用时间长,具有较高的应用价值,值得推广。Objective To observe the long - term effects of Liver - smoothing and heat - clearing decoction on the treatment of polycystic ovarian syndrome（PCOS）with heat stagnated in liver meridian type in adolescence. Methods 86 PCOS patients in adolescence with heat stagnated in liver meridian type were randomly divided into two groups. The control group（n = 43）received ethinylestradiol and cyproterone acetate tablets,and the treatment group received Liver - smoothing and heat - clearing decoction,continuously treatment for 6 menstrual cycles. The body mass,modified Ferriman - Gallwey（mFG）,ovarian volume,sex hormone including testosterone（T）,follicle stimulating hormone（FSH）,luteinizing hormone（LH）of six months after withdrawal were observe in two groups. The recovery condition of menstrual cycle was recorded. Results The total effective rate in treatment group （95. 3% ）was obviously higher than control group（74. 4% ,P 〈 0. 05）. The body mass,body mass index（BMI）, mPG score,ovarian volume,T,LH,LH/ FSH of six months after withdrawal in treatment group were lower than those in control group（P 〈 0. 05）,but there were no statistical differences on FSH between two groups（P 〈 0. 05）. The recovery rate of menstrual cycle in treatment group（90. 7% ）was obviously higher than control group（55. 8% ,P 〈0. 05）. Conclusion Liver - smoothing and heat - clearing decoction has exact effects on the treatment of adoles-cence PCOS with heat stagnated in liver meridian type,can effectively improve the clinical symptoms and the hormone concentration,with more persistent effect and superior application value,worthy of clinical application.
