风湿6号膏方对急性痛风性关节炎患者外周血白细胞介素1及肿瘤坏死因子α的影响

摘　　要：目的观察风湿6号膏方治疗急性痛风性关节炎的临床疗效及对外周血中白细胞介素1(IL-1)和肿瘤坏死因子α(TNF-α)的影响。方法将120例急性痛风性关节炎患者随机分为2组,每组各60例。对照组予塞来昔布胶囊、秋水仙碱片口服治疗,治疗组在对照组基础上予风湿6号膏方内服治疗。比较2组前后临床症状、体征积分及血尿酸、红细胞沉降率、C反应蛋白、IL-1及TNF-α水平变化,统计临床总体疗效。结果治疗组总有效率为83.33%,对照组为66.67%,2组总有效率比较差异有统计学意义(P<0.05),治疗组临床疗效优于对照组。2组治疗后关节肿胀、疼痛及活动受限评分均较本组治疗前明显降低,且治疗组低于对照组,比较差异均有统计学意义(P<0.05)。2组治疗后血尿酸、红细胞沉降率、C反应蛋白水平及IL-1、TNF-α水平均较本组治疗前明显下降(P<0.05),且治疗组低于对照组(P<0.05)。结论风湿6号膏方治疗急性痛风性关节炎疗效显著,降低外周血中IL-1和TNF-α的表达水平可能是其抗炎、镇痛的作用机制之一。Objective To observe the clinical effects of Reumatoid 6 Paste on the treatment of acute gouty arthritis and the effects of peripheral serum interleukin - 1（ IL - 1）and tumor necrosis factor - α（ TNF - α）. Methods 120 patients with acute gouty arthritis were randomly divided into two groups,60 cases in each group. The control group received celecoxib capsule and colchicine tablets,and the treatment group received Reumatoid 6 Paste on the basis of control group treatment. The integrals of clinical symptoms and signs,blood uric acid（BUA）, erythrocyte sedimentation rate（ESR）,C - reactive protein（CRP）,IL - 1 and TNF - α before and after treatment were observed in two groups,and the curative effect was evaluated. Results The total effective rate in treatment and control group was 83. 33% and 66. 67% ,respectively,with statistical differences（P 〈 0. 05）. The curative effect in treatment group was superior to that in control group. The joint swelling,pain and the function of joint after treatment were obviously improved in two groups（P 〈 0. 05）,and the improvements in treatment group were superior to those in control group（P 〈 0. 05）. The levels of BUA,ESR,CRP,IL - 1 and TNF - α after treatment were obviously decreased in two groups（P 〈 0. 05）,and the levels in treatment group was lower than control group（P 〈0. 05）. Conclusion Reumatoid 6 Paste has significant effects on the treatment of acute gouty arthritis,can decrease the expression of peripheral serum IL - 1 and TNF - α which maybe one of the action mechanism of anti - inflammatory and analgesic.
