甲钴胺和硫辛酸治疗糖尿病周围神经病变患者的临床效果及作用机制

摘　　要：目的探讨甲钴胺联合硫辛酸治疗糖尿病周围神经病变（diabetic peripheral neuropathy,DPN）患者的临床效果。方法选取2015年1月-2016年3月中北大学医院诊治的90例DPN患者采用抽签法随机分为联合组和甲钴胺组各45例,2组均予以甲钴胺治疗,联合组同时给予α-硫辛酸治疗,连续治疗4w后观察临床效果。结果治疗后,联合组的肢体疼痛、局部压痛、肢端感觉异常、手足畏寒、腱反射评分显著的低于甲钴胺组（P〈0.05）;联合组患者两侧胫神经的运动神经传导速度（motor nerve conduction velocity,MNCV）、感觉神经传导速度（sensory nerve conduction velocity,SNCV）测定值均大于甲钴胺组患者,差异具有统计学意义（P〈0.05）;联合组患者血清同型半胱氨酸（Hcy）、C反应蛋白（CRP）测定值均低于甲钴胺组患者,差异具有统计学意义（P〈0.05）;联合组患者总有效率91.11%高于甲钴胺组患者的75.56%,差异具有统计学意义（P〈0.05）。结论甲钴胺联合硫辛酸治疗DPN患者的疗效优于单用甲钴胺治疗的临床效果。Objective To explore the clinical effect mecobalamin combined with lipoic acid in the treatment of diabetic peripheral neuropathy of（ DPN）. Methods From January 2015 to March 2016,90 cases of DPN patients in North Central University Hospital were randomly divided into combination group and mecobalamin group with 45 cases in each group,two groups were treated with mecobalamin,combination group were given alpha lipoic acid treatment,the clinical effect was observed after 4 weeks＇ treatment. Results After treatment,the combination group of limb pain and local tenderness,acroparesthesia,chills,foot tendon reflexes were lower than those of mecobalamin group significantly（ P〈0. 05）; motor nerve conduction velocity（ MNCV）,sensory nerve conduction velocity（ SNCV） values of the bilateraltibial nerve in combination group were greater than mecobalamin group patients（ P〈0. 05）; the serum Hcy,CRP in combination groupwere lower than mecobalamin group（ P〈0. 05）; the total effective rate in combination group（ 91. 11%） higher than that of mecobalamin group（ 75. 56%）（ P〈0. 05）. Conclusion The clinical efficacy of mecobalamin cmbined with lipoic acid in the treatment of patients with DPN is better than single mecobalamin.
