氨溴索联合沙丁胺醇对胸食道手术患者围手术期的应用效果及对肺功能的影响

摘　　要：目的探讨氨溴索联合沙丁胺醇对胸食道手术患者围手术期的应用效果及对肺通气和血清炎症细胞因子水平的影响。方法回顾性分析2014年5月-2016年8月在浙江省绍兴市人民医院接受胸食道手术的患者的临床资料,根据其围手术期雾化吸入药物的不同分为氨溴索组（对照组）和氨溴索联合沙丁胺醇组（观察组）。观察比较2组手术前后肺功能指标、炎症细胞因子、血气指标和睡眠质量的差别。结果手术后,2组FVC、FEV1、FEV1/FVC、MMEF和PEF水平显著降低,与对照组比较,观察组较高;手术后,观察组患者的IL-18、IL-6和hs-CRP水平显著低于对照组（P〈0.05）;2组患者手术前血气指标无差别,手术后,观察组的Pa CO2低于对照组（P〈0.05）,PaO2和PaO2/FiO2水平高于对照组（P〈0.05）;观察组患者的睡眠质量明显优于对照组（P〈0.05）。结论氨溴索联合沙丁胺醇可明显改善胸食道手术患者术后肺功能和血清细胞因子水平,具有临床应用价值。Objective To explore the clinical effect of ambroxol combined with salbutamol in patients with thoracic esophageal surgery and the effect on lung function and serum levels of inflammatory cytokines. Methods Retrospective analysis was used to analysis the clinical data of patients accepted the chest esophagus surgery in our hospital from May 2014 to August 2016,according to the perioperative inhaled drugs were divided into ambroxd group（ control group） and salbutamol and ambroxol group（ observation group）. The pulmonary function indexes and inflammatory cytokine levels were observed before and after operation in two groups. The differences of sleep quality between the two groups before and after operation were compared. Results After surgery,the level of FVC,FEV1,FEV1 / FVC,MMEF and PEF of the two groups all decreased,compared with the control group,the observation group was higher. The level of inflammatory cytokines in the two groups had no difference. After surgery,the level of IL-18,IL-6and hs-CRP of the observation group were significantly lower than those in the control group（ P〈0. 05）; The level of blood gas indexes in the 2 groups before operation had no differences. After surgery,the level of Pa CO2 in the observation group was lower than that of the control group（ P〈0. 05）,the level of PaO2 and PaO2/ FiO2 were higher than those in the control group（ P〈0. 05）. Conclusion The application of ambroxol combined with salbutamol in patients with thoracic esophageal surgery,can significantly improve the postoperative pulmonary function and serum cytokine levels,has clinical application value.
