固相凝集法血小板抗体检测与早期流产相关性探讨

摘　　要：目的研究探讨通过固相凝集法检测血小板抗体对判断早期流产的临床意义,并分析血小板抗体检测结果与早期流产的相关性。方法选取江西省赣州市人民医院产科2015年9月-2016年6月就诊的有早期流产史的50例女性作为研究对象,采用固相凝集法对其血小板抗体进行检测,并计算检测阳性率。同时,选取同期在产科接受产前检查的正常孕妇1092例作为对照,行相同检测,比较2组受检女性的血小板抗体检测阳性率。结果有早期流产史的观察组女性血小板抗体阳性检出率（22.0%）显著高于无早期流产史的正常孕妇（3.1%）（P〈0.05）;有早期流产史的女性中,流产次数〉2次的女性血小板抗体筛查阳性率（40.0%）显著高于流产次数≤2次的女性（17.5%）（P〈0.05）;血小板抗体检测阳性的女性早期流产的发生率（24.4%）显著高于检测阴性的女性（3.6%）（P〈0.05）,且血小板抗体筛查结果与早期流产之间存在显著相关性（r=0.782,P〈0.05）,其相对风险RR=6.96。结论通过固相凝集法对孕妇的血小板抗体进行筛查,有利于早期流产发生风险的预测,也有利于流产病因的分析及新生儿同种免疫性血小板减少性紫癜的预防。Objective To investigate the clinical significance of platelet antibodies in the determination of early abortion by solid-phase agglutination,and to analyze the correlation between the results of platelet antibody detection and early abortion. Methods 50 women selected from Ganzhou People＇s Hospital of Jiangxi province from September 2015 to June 2016 had early abortion history as the research object,using solid agglutination method to detect platelet antibody,and calculate the positive detection rate. At the same time,1092 cases pregnant women undergoing normal prenatal examination were selected as control group,for the same detection,compare platelet antibody positive rate between two groups. Results The platelet antibody positive rate in early history of abortion in observation group female was 22%,which was significantly higher than that in normal pregnant women without early abortion（ 3. 1%）（ P〈0. 05）; the platelet antibody screening positive rate in abortion time above twice of early abortion women was 40. 0%,significantly higher than bellow twice（ 17. 5%）（ P〈0. 05）; the early abortion rate of platelet antibody positive women was 24. 4%,significantly higher than that of negative women（ 3. 6%）,there were statistically significant differences（ P〈0. 05）,and there was a significant correlation between the platelet antibody screening results and early abortion（ r = 0. 782,P〈0. 05）,the relative risk of RR = 6. 96. Conclusion It is helpful to predict the risk of early abortion and the analysis of the cause of abortion and the prevention of neonatal autoimmune thrombocytopenia purpura by the method of solid state agglutination.
