子宫动脉栓塞术在前置胎盘孕妇引产前应用的疗效观察

摘　　要：目的 评估子宫动脉栓塞术（UAE）在前置胎盘孕妇引产前应用的疗效。方法 选取2011年引产合并前置胎盘，患者及其家属实验组引产前行双侧UAE，行利凡诺羊膜腔注射＋米非司酮药物引产对照组单纯行利凡诺羊膜腔注射＋米非司酮药物引产。比较两组引产过程中出血量、产后出血量、全子宫切除的发生率，两组患者随访1-2年，比较两组患者再次妊娠的成功率，评估UAE在前置胎盘孕妇引产前应用的疗效。结果 实验组引产过程中出血量明显低于对照组（±29.98 vs. 134.33±17.72，t=2.308，P＜0.05），实验组引产后出血量明显低于对照组（112.41±20.70 vs. 138.00±32.50，t=2.057，P＜0.05），实验组引产后刮宫出血量明显低于对照组（69.17±26.16 vs. 93.50±12.03，t=5.582，P＜0.05），实验组子宫切除的发生率与对照组相比差异无统计学意义（χ^2=0.156，P＞0.05），再次妊娠的成功率，两组比较差异无统计学（χ^2=0.396，P＞0.05）。结论 合并前置胎盘的孕妇引产前行UAE能减少引产过程中及产后的出血量，减少引产后刮宫过程中的出血量，而且对患者的生育能力影响较小。Objective To evaluate the curative effect of Uterine artery embolization（UAE）in gravidas with placenta previa before the odinopoeia.Methodsodinopoeia with placenta previa, divide into two groups according to the wishes of patients and their families, who take UAE before the odinopoeia in 2011 as experimental group, who only take Rivanol and Mifepristone in their odinopoeia as contrast group. Comparing the blood loss in the process of odinopoeia、Postpartum and The incidence of total hysterectomy between the two groups. All of the patients in our study were followed-up for 1-2 years,and make a comparison between the two groups in success rate of pregnancy again. Results The blood loss in the process of odinopoeia in experimental group was Significantly lower than contrast group （ml： 109.65±29.98 vs. 134.33±17.72,t=2.308,P〈0.05）,and the same result was appeared in Postpartum blood loss（112.41±20.70 vs. 138.00±32.50,t=2.057,P〈0.05）、curettage blding （ml：69.17±26.16 vs. 93.50±12.03,t=5.582,P〈0.05）.The diference of total hysterectomy rateχ^2=0.156,P〉0.05 and successful pregnancy again rate（29.4% vs. 40.0%,χ^2=0.396,P〉0.05 between them have no statistical significance. ConcIusions It can reduce the blood loss in the process of odinopoeia、postpartum for gravidas and curettage with placenta previa to take a UAE before odinopoeia. And smaller effects on fertility in patients.
