头位妊娠脐带绕颈252例临床分析

摘　　要：目的:探讨头位妊娠脐带绕颈对分娩方式和新生儿的影响。方法:回顾性分析头位妊娠脐带绕颈产妇252例的临床资料,分析脐带绕颈周数、分娩方式以及对新生儿的影响。结果:脐带绕颈1周和脐带绕颈2周在胎儿窘迫、新生儿窒息、吸入性肺炎、新生儿死亡方面比较,差异无统计学意义(P>0.05)。脐带绕颈3周的新生儿出现新生儿窒息1例、吸入性肺炎1例,死亡1例。自然分娩和胎吸助产的新生儿窒息发生率明显高于急诊剖宫产,差异具有统计学意义(P<0.05)。胎吸助产死亡率明显高于自然分娩和急诊剖宫产,差异具有统计学意义(P<0.05)。结论:剖宫产可以明显降低脐带绕颈对新生儿的影响。脐带绕颈周数越多,发生胎儿窘迫、新生儿窒息、吸入性肺炎、新生儿死亡的风险越大。ObjectiveTo investigate the effect of umbilical cord entangle neck with head delivery on the delivery mode and thenewborns.Methods：The clinical data of 252 cases of puerperae with umbilical cord entangle neck and head delivery were analyzedretrospectively.The effect of the laps of umbilical cord entangle neck,delivery mode on the newborns were analyzed.Results：Therewere no statistically significant differences of fetal distress,neonatal asphyxia,aspiration pneumonia and neonatal death betweenone lap and two laps of umbilical cord entangle neck （P〉0.05）.The newborns with three laps of umbilical cord entangle neck had1 case of neonatal asphyxia and 1 case of aspiration pneumonia,with 1 cases of death.The incidence of neonatal asphyxia in thenewborns with natural childbirth and fetal aspiration midwifery were significantly higher than that of emergency cesarean section,and the differences were statistically significant （P〈0.05）.The mortality of the newborns with fetal aspiration midwifery wassignificantly higher than that of natural childbirth and emergency cesarean section,and the difference was statistically significant（P〈0.05）.Conclusion：Cesarean section could reduce the effect of umbilical cord entangle neck on the newborn significantly.Themore the laps of umbilical cord entangle neck,the greater the risk of fetal distress,neonatal asphyxia,aspiration pneumonia andneonatal death.
