小剂量氟哌利多联合异丙酚对等效剂量芬太尼和舒芬太尼诱发呛咳的影响

摘　　要：目的探讨小剂量氟哌利多联合异丙酚对等效剂量芬太尼和舒芬太尼诱发呛咳的影响。方法选择200例拟行气管插管全身麻醉的择期手术患者。按美国麻醉医师协会的分级,随机分成4组,每组50例。对照组(A1、A2组)全麻诱导前输注生理盐水0.15 ml/kg+异丙酚1 mg/kg。氟哌利多联合异丙酚治疗组(B1、B2组)全麻诱导前输注稀释的2μg/kg氟哌利多+异丙酚1 mg/kg(把2μg/kg的氟哌利多稀释到0.15 m1/kg的生理盐水中)。1 min后,A1、B1组静脉5 s注射芬太尼4μg/kg,A2、B2组5 s注射舒芬太尼0.5μg/kg,记录注射芬太尼或舒芬太尼后1 min内呛咳的发生情况和强度。结果与A1组和A2组比较B1组和B2组呛咳的发生率显著降低(P<0.01),A1组和A2组比较B1组和B2组呛咳程度,差异有统计学意义。结论静脉注射氟哌利多2μg/kg联合异丙酚1 mg/kg可完全抑制等效剂量芬太尼或舒芬太尼诱发的呛咳。Objective To investigate the effect of low-dose droperidol combined with propofol on cough induced by an equivalent dose of fentanyl and sufentanil. Methods 200 patients aged 18 to 65 who had been given general anesthesia for tracheal intubation were selected and randomly divided into 4 groups （50 cases each） according to the American Society of Anesthesiologists grading. The control groups （group A1 and A2） were infused with propofol 1 mg/kg and normal saline 0.15 ml/kg before induction of anesthesia while droperidol and propofol treatment groups （group B1 and B2） were infused with diluted droperidol 2 μg/kg and propofol 1 mg/kg before induction of anesthesia. After 1 min, group A1 and B1 received intravenous injection of fentanyl 4 μg/kg, while group A2 and B2 was injected with sufentanil 0.5 μg/kg before the occurrence and intensity of choking cough 1 min after injection were recorded. Results Compared with group A1 and group B1, the incidence of cough in group B2 and group A2 was significantly decreased （P〈0.01）. F test showed that there was significant difference between group A1, group A2and group B1 and group B2. Conclusion Intravenous injection of droperidol 2 μg/kg combined with propofol 1 mg/kg can completely inhibit cough induced by an equivalent dose of fentanyl or sufentanil
