摘  要:目的采用3.0 T磁共振定量分析冰雪运动对儿童及青少年膝关节软骨发育的影响。方法吉林大学中日联谊医院2014年6月至2015年6月招募20名参加冰雪运动的儿童及青少年与20名普通健康的儿童及青少年,分为冰雪运动组与对照组,进行3.0 T膝关节磁共振扫描,根据膝关节承重及冰雪运动的应力分布特点对膝关节股骨内侧髁、股骨外侧髁、胫骨内侧平台、胫骨外侧平台4个部位进行分区处理,对各个区域的软骨厚度、T2值进行定量测量。结果冰雪运动组年龄11-17岁,平均年龄14.2岁,体质指数(BMI)为18.3 kg/m^2;对照组年龄10-18岁,平均年龄13.6岁,BMI为17.9 kg/m^2,差异均无统计学意义(均P〉0.05)。在冰雪运动组与对照组中,男性与女性的股骨内、外侧髁的中央前区、中央后区、后区前部以及胫骨平台内、外侧的前、中、后区的软骨厚度值差异均有统计学意义(均P〈0.05)。冰雪运动组与对照组股骨内、外侧髁的中央前区、中央后区、后区前部以及胫骨平台内、外侧的前、中、后区的软骨厚度值差异均有统计学意义(均P〈0.05)。在冰雪运动组与对照组中,男性与女性的股骨内、外侧髁的中央前区、中央后区、后区前部以及胫骨平台内、外侧的前、中、后区的软骨的T2值差异均有统计学意义(均P〈0.05)。冰雪运动组与对照组股骨内、外侧髁软骨的中央前区、中央后区及胫骨平台的内侧软骨的前、中、后区软骨的T2值差异均无统计学意义(均P≥0.05),而股骨内、外侧髁软骨的股骨后区前部、胫骨平台内、外侧软骨的后区软骨的T2值差异均有统计学意义(均P〈0.05)。 结论性别差异是软骨厚度的重要影响因素,科学恰当的冰雪运动对青少年的膝关节软骨发育可能存在促进作用。ObjectiveTo evaluate the effect of snow sports to the knee cartilage maturation of children and adolescent with quantitative analysis of 3.0 T MRI.MethodsTwenty children and adolescent who underwent snow sports training more than 6 months (11 males and 9 females) were recruited and named with sports group, and twenty children and adolescent who live regularly without any exercise (10 males and 10 females) were recruited as control group from June 2014 to June 2015.Each group was divided into subgroups according to their gender.Both groups were scanned by 3.0 T MR.According to the loading features of knee articular cartilage in snow sports, medial femoral condyle, lateral femoral condyle, medial tibial condyle and lateral tibial condyle were divided into subareas.T2 values and cartilage thickness were measured in all areas.ResultsThe median ages of the sports group and control group were 14.2 and 13.6 years old, which showed no statistical difference between two groups.The BMI of the sports group and control group were 18.3 and 17.9 kg/m^2, which showed no statistical difference between two groups.Both in sports group and control group, the thickness and T2 values of all areas showed significant differences between man and women in each group (all P〈0.05). The cartilage thickness of sports group was higher than control group (P〈0.05). The T2 values showed no statistical difference in anterior central, posterior central of femoral condyles and anterior, central areas of tibial condyles between sports group and control group(P≥0.05). The T2 values of anterior part of posterior femur and posterior areas of tibial condyle in sports group was higher than control group (P≥0.05).ConclusionGender difference may be an important effect factor of cartilage thickness. Scientific snow sports training maybe has positive influence for cartilage maturation of children and adolescent.
