主动脉夹层患者脂蛋白相关磷脂酶A_2浓度水平及临床意义的研究

摘　　要：主动脉夹层（aortic dissection,AD）在急性期致残率及病死率高,是一种灾难性血管性疾病,因此早期确诊极其重要。目前,其诊断主要依靠影像学检查,尚缺乏特异性较强的血液检查指标。因而,寻找一种方便且敏感度较高的诊断筛选方法显得尤为迫切。高血压、动脉粥样硬化均是AD的重要病因,而脂蛋白相关磷脂酶A2（Lp-PLA2）是近年来炎症反应介质的热点,其具有促进动脉粥样硬化作用,且与高血压病密切相关,而Lp-PLA2与AD之间是否存在联系目前尚无报道。为此笔者检测358例急性胸背痛患者血清中Lp-PLA2水平,探讨其对AD的诊断预测价值。To study the value of lipoprotein-associated phospholipase A2（Lp-PLA2）in the early diagnosis of aortic dissection（AD）.186 patients with AD（AD group）admitted to our hospital and 172 other patients with chest pain（control group）at the same time were selected.The difference in the level of Lp-PLA2 between the two groups was compared,and the sensitivity,specificity,positive predictive value and negative predictive value of Lp-PLA2 to the diagnosis of AD were analyzed using the ROC curve.The level of Lp-PLA2 in the AD Group was significantly higher than that in the control group,and the difference was statistically significant（P〈0.01）.During the ROC analysis,when Lp-PLA2 was valued at 60.2μg/L,the sensitivity,specificity,positive predictive value and negative predictive value of Lp-PLA2 to the diagnosis of AD were 82.2%,70.4%,81.5%and 96.1%,respectively.The detection of Lp-PLA2 provided a positive reference for the early diagnosis and treatment of patients with AD.
