64排螺旋CT评价主动脉夹层肾缺血的初步研究

摘　　要：目的 评价64排螺旋CT对主动脉夹层肾动脉受累时肾功能的应用价值。方法 回顾性分析57例发生主动脉夹层并且肾动脉受累患者的临床及CT资料,分析其双侧肾脏增强扫描时同区域CT值差异,评价患者肾功能情况。结果 57例患者中,双侧肾脏同区域平均CT值差值0-30 HU者23例,患者均无肾功能异常者;平均CT值差值30~60 HU者26例,其中肾功能异常者5例（19.2%）;平均CT值差值60-90 HU者8例,其中肾功能异常5例（62.5%）。结论 主动脉夹层患者肾动脉受累时肾血供相应减少,可发生急性肾功能不全。64排螺旋CT可以通过测量双侧肾脏同区域CT值的差值评价患者肾功能情况。Objective To evaluate 64-slice spiral CT in diagnosis of renal function in aortic dissection-involved renal artery. Methods Clinical data and CT images of 57 cases with aortic dissection-involved renal artery were analyzed retrospectively. Differences of CT value in same ROI of both kidneys and renal function was evaluated in enhanced CT scans. Results In 57 cases, 23 cases showed that differences in average CT value ranged from 0 - 30 HU and renal function was normal （0%）. 26 cases showed that differences in average CT value ranged from 30 -60 HU and 5 cases showed abnormal renal function （ 19.2% ）. 8 cases showed differences of average CT value ranged from 60 - 90 HU and 5 cases showed abnormal renal function （62.5%）. Conclusion Renal blood supply was reduced after aortic dissection-involved renal artery and acute renal insufficiency could be developed secondarily. 6d-slice spiral CT was able to evaluate the differences of CT value in same ROI of both kidney and assessment of renal function.
