β2-微球蛋白对IgA肾病肾小管间质损伤的评价作用

摘　　要：目的:探讨β2-微球蛋白(β2-MG)在评估IgA肾病(IgAN)患者肾小管间质损伤中的价值。方法:选取IgAN患者201例,免疫比浊法测定血、尿β2-MG;肾小管间质病变评分参照Katafuchi半定量积分标准。结果:血、尿β2-MG随IgAN肾小管间质损伤的加重呈明显升高趋势,血、尿β2-MG与肾小管间质损伤间均呈显著正相关关系(P<0.01)。血β2-MG水平正常(<2.2 mg/L)时,95.6%患者存在肾小管间质病变,其中13.2%患者病变已达中度,尿β2-MG与肾小管间质损伤无相关关系(P>0.05)。结论:血β2-MG水平异常升高时,对IgAN肾小管间质损伤的评估价值较大。Objective：To investigate the value of β2-microglobuin （β2-MG ） in the evaluation of tubulointerstitial injury in IgA nephropathy（IgAN）. Methods：The levels ofβ2-MG in blood and urine and scores of tubulointerstitial injury in 201 IgAN patients were investigated using the immunoturbidimetric method and Katafuchi semi-quantitative standard,respectively. Results：With the aggravation of tubulointerstitial injury,the levels ofβ2-MG in blood and urine increased significantly. The levels ofβ2-MG in blood and urine were positive correlation with the tubulointerstitial injury（P 〈0. 01）. When the level of serum β2-MG was normal（ 〈2. 2 mg/L）,the tubulointerstitial lesion was found in 95. 6% of patients,which included the moderate tubulointerstitial lesion in 13. 2% of patients,and the level ofβ2-MG in urine was not correlation with the tubulointertitial lesion（P〉0. 05）. Conclusions：The abnormal increasing of the serum level of β2-MG has great value in evaluating the tubulointerstitial injury of IgA nephropathy.
