曲伏前列腺素滴眼液对原发性开角型青光眼患者眼压及血浆内皮素-1的影响

摘　　要：目的观察曲伏前列腺素滴眼液对原发性开角型青光眼患者眼压及血浆中内皮素^(-1)的影响。方法 80例原发性开角型青光眼患者随机分为试验组40例和对照组40例。对照组给予马来酸噻吗洛尔滴眼液,每次1滴,早晚各1次。试验组给予曲伏前列腺素滴眼液,每次1滴,睡前1次。2组均长期持续治疗。比较2组治疗前和治疗后3个月的眼压及血浆中内皮素^(-1)的变化情况。结果治疗后,试验组的平均眼压为(17.12±1.80)mm Hg,对照组为(19.02±1.55)mm Hg(P<0.05)。治疗后,试验组的前后昼夜眼压差为(3.92±0.89)mm Hg,对照组为(4.55±0.96)mm Hg(P<0.05)。治疗后,试验组视网膜中央动脉(CRA)的收缩期峰速(PSV)为(10.79±2.15)cm·s^(-1),舒张末期流速(EDV)为(3.75±0.74)cm·s^(-1),阻力指数为0.61±0.05;对照组的PSV为(8.67±1.77)cm·s^(-1),EDV为(2.71±0.72)cm·s^(-1),阻力指数为0.67±0.12,2组差异有统计学意义(P<0.05)。治疗后,试验组睫状后动脉(PCA)的PSV为(13.23±3.16)cm·s^(-1),EDV为(3.75±0.70)cm·s^(-1),阻力指数为0.65±0.07;对照组的PSV为(9.91±2.56)cm·s^(-1),EDV为(2.72±0.73)cm·s^(-1),阻力指数为0.74±0.10,2组差异有统计学意义(P<0.05)。治疗后,试验组血浆内皮素^(-1)水平为(45.22±7.56)ng·L^(-1),对照组为(49.32±8.05)ng·L^(-1)(P<0.05)。对照组出现眼部不适3例,眼部轻度干涩5例,药物不良反应发生率为20.00%(8/40例);试验组出现睫毛增粗2例,眼红4例,眼部轻度干涩4例,药物不良反应发生率为25.00%(10/40例),差异无统计学意义(P>0.05)。结论曲伏前列腺素对原发性开角型青光眼患者的临床疗效良好,可以降低患者的眼压和患者血浆内皮素^(-1)水平。Objective To evaluate the influence of travoprost on in- traocular pressure and plasma endothelin -1 in patients with primary open- angle glaucoma. Methods Eighty patients with primary open - angle glaucoma were randomly divided into treatment group （n = 40） and control group （n = 40）. Control group was treated with timolol maleate eye drops, 1 drop in morning and evening respectively. Treatment group was given travoprost ophthalmic solution, 1 drop before bedtime. The course of treatment was a long period. The changes of intraocular pres- sure （IOP） and plasma endothelin - 1 in two groups were compared be- fore treatment and after 3 months treatment. Results The IOP in treat- ment group was （17.12 ± 1.80） mmHg, had significant difference withcontrol group, which was （19.02 ± 1.55 ） mmHg（ P 〈 0.05 ）. The diurnal intraocular pressure difference in treatment group was （3.92 ± 0. 89 ） mmHg, had significant difference with control group, which was （4. 55 ± 0. 96 ） mmHg （ P 〈 0. 05 ）. Alter treatment, the peak systolic relocit （ PSV）, end - diastolic velocity （ EDV ） and resistance index （RI） in central retinal artery （CRA） were （10.79 ±2. 15） cm . s-1, （3.75 ±0.74） cm . s-1, 0.61 ±0.05 intreat- ment group ; had significant difference with control group, which were （ 8.67 ± 1.77 ） cm . s-1, （ 2.71 ± 0. 72 ） cm.s-1 , 0.67 ±0.12（P 〈0.05）. The PSV, EDV and RI in principal components analysis （PCA） were （13.23 ±3.16） cm . s-1 （3.75 ±0.70） cm . s-1 0.65 ±0.07; had significant difference with control group which were （ 9. 91± 2. 56 ） cm.s-1, （2.72±0.73） cm.s-1, 0.74±0.10 （P〈0.05）. The level of plasma endo- thelin- 1 in treatment group was （45.22 ±7.56） ng . L-1, had significant difference with （49.32 ±8.05） ng . L-1 in control group （P 〈 0. 05 ）.
