大剂量泼尼松不同治疗时间对胶原诱导性关节炎大鼠骨骼的影响

摘　　要：目的探讨大剂量泼尼松不同治疗时间对胶原诱导性关节炎(CIA)大鼠骨骼的影响。方法 50只雌性大鼠随机选取12只为正常对照组,其余建立CIA模型。免疫4周后筛选出CIA大鼠24只,随机均分为CIA模型组和泼尼松组,于治疗30、90 d后各组分别处死6只大鼠取材,进行血清骨转换生化标志物、腰椎骨密度和胫骨上段松质骨形态计量学的测量。结果与正常对照组比较,CIA模型组腰椎骨密度和胫骨上段的骨量均减少(P<0.01)。与CIA模型组比较,泼尼松组胫骨上段的骨量均有所增加(P<0.05),但腰椎的骨密度无明显变化(P>0.05)。此外,泼尼松组治疗90 d后血清骨保护素和Ⅰ型前胶原氨基端原肽均降低(P<0.05)。结论大剂量泼尼松治疗30和90 d均可延缓CIA大鼠的骨丢失,但不能降低骨折的危险性。泼尼松治疗90 d对CIA大鼠骨代谢有负性影响。Objective To investigate the effects of high - close prednisone treatment for different duration on bone metabolism in collagen - induced arthritis （CIA） rats. Methods Fifty female rats were randomly divided into control group （n = 12）, CIA group （n =24） and prednisone group （n =24）. Rats in control and CIA group were given with saline by 5 mL/kg/d, while rats in prednisone group were given with prednisone by 4. 5 mg/kg/d. Six rats were killed in each group on the 30 d and 90 d. Serum bone turnover markers, bone mineral density （BMD） of fifth lumbar vertebrae （ LV5 ） and bone histomorphometry of the proximal tibial metaphysis （ PTM ） were measured. Results Compared with control group, BMD of LV5 and bone mass of PTM were decreased in CIA group （ P 〈 0.05 ）. Compared with CIA group, bone mass of PTM was increased in prednisone group, but there was no significant change in BMD of LV5 （ P 〉 0. 05 ）. In addition, the serum OPG and PINP were significantly decreased in CIA rats treated with prednisone for 90 days （P 〈 0. 05 ）. Conclusion High - close preclnisone treatment for 30 clays and 90 days could prevent bone loss in CIA rats, but could not reduce the risk of fracture. High - dose prednisone treatment for 90 days showed negative effect on bone metabo- lism in CIA rats.
