强化瑞舒伐他汀治疗对老年冠心病患者颈动脉斑块和血清IL6及hs-CRP的影响

摘　　要：目的探讨强化瑞舒伐他汀对老年冠心病患者血清白细胞介素6(IL-6)和超敏C反应蛋白(hs-CRP)以及颈动脉斑块的影响。方法将125例经冠脉造影确诊为冠心病的老年患者随机分成两组,对照组(n=60例)常规每晚口服瑞舒伐他汀10mg,强化组(n=65例)每晚口服瑞舒伐他汀1次,每次20mg,治疗1个月前后测定血清IL-6及hs-CRP,治疗6个月前后用彩色多普勒超声诊断仪检查颈动脉内膜的厚度及颈动脉斑块的变化。结果强化组患者治疗1个月后,血浆中的hs-CRP和IL-6水平显著低于治疗前水平及对照组水平(P<0.01);6个月后,强化组患者双侧颈动脉内膜厚度明显降低及颈动脉斑块面积明显缩小,与治疗前及对照组比较有显著性差异(P<0.01)。结论瑞舒伐他汀有效降低IL-6及hs-CRP的水平,改善老年冠心病患者颈动脉内膜状态,使颈动脉斑块缩小。Objectives To investigate the effects of intensive rosuvastatin on arotid atherosclerotic plaques and hs-CRP IL6 in old patients with coronary heart disease. Methods 125 aged patients with CAD were randomly divided into two groups, the intensive group（n=65） was treated with normal therapy and rosuvastatin 10 mg, the control group（n=60） was treated with normal therapy. The serous high sensitive C-reactive protein（hs-CRP） and interleukin 6（IL-6） were monitored before and after treatment for one month, the carotid atherosclerotic plaques were assessed by ultrasonography before and after treatment for six months. Results After 1 month of treatment, the levels of hs-CRP and IL-6 were significantly decreased compared with the control group and before the treatment in the intensive rosuvastatin group（P〈0.01）. After 6 months, the intima thickness of bilateral carotid artery was significantly decreased as well as the carotid artery plaque area was obviously smaller in the intensive rosuvastatin group, and had significantly difference（P〈0.01）. Conclusions Intensive rosuvastatin could significantly decrease the level of hs-CRP and IL-6, improve the carotid intima state of aged patients with coronary heart disease（CHD）, and decrease the carotid atherosclerotic plaques.
