摘  要:目的研究干扰素-r(IFN-r)和腺苷脱氨酶(ADA)水平对结核性和癌性胸腔积液的诊断价值。方法选取2014年5月—2015年5月河北省胸科医院收治的胸腔积液患者118例,其中结核性胸腔积液59例(结核组),癌性胸腔积液28例(癌性组),炎性胸腔积液22例(炎性组),其他原因引起胸腔积液9例(其他组),检测4组胸腔积液中的INF-r和ADA水平,并进行比较。结果结核组INF-r、ADA水平均高于癌性组、炎性组及其他组(P<0.05);癌性组INF-r、ADA水平均高于炎性组及其他组(P<0.05)。INF-r+ADA联合诊断结核性胸腔积液较单独诊断的特异度高,差异具有统计学意义(P<0.05)。结论 INF-r和ADA水平对结核性和癌性胸腔积液有诊断价值,且能鉴别结核性和癌性胸腔积液,两者联合更有助于结核性胸腔积液的诊断。Objective To study value of interferon-γ(INF-r) and adenosine deaminase(ADA) levels in diagnosis of tuberculous and malignant pleural effusion.Methods A total of 118 patients with pleural effusion admitted during May 2014 and May 2015 were divided into tuberculous pleural effusion(TB group,n = 59),malignant pleural effusion(cancer group,n = 28),inflammatory pleural effusion(inflammatory group,n = 22) and other causes of pleural effusion groups(other group,n = 9).INF-r and ADA levels were detected and compared in four groups.Results INF-r and ADA levels in TB group were higher than those in cancer,inflammatory and other groups(P〈0.05).INF-r and ADA levels in cancer group were higher than those in inflammatory and other groups(P〈0.05).Specificity degree of INF-r combined with ADA was higher than that of separate one in diagnosis of tuberculous pleural effusion,and the difference was statistically significant(P〈0.05).Conclusion INF-r and ADA levels have value in diagnosis of tuberculous and malignant pleural effusion,and it can identify tuberculous and malignant pleural effusion,and INF-r combined with ADA is more helpful in diagnosis of tuberculous pleural effusion.
