探讨地塞米松联合尿激酶治疗结核性胸膜炎的临床效果

摘　　要：目的 探讨地塞米松联合尿激酶治疗结核性胸膜炎的临床效果。方法 选取2014年1月～2016年1月我院收治结核性胸膜炎患者50例,根据随机方法将其分为对照组与观察组,各25例,对照组予以胸腔内注入地塞米松治疗,观察组予以地塞米松联合尿激酶注入胸腔治疗,对比两组患者的治疗有效率、不良反应发生率、胸水排除总量、胸膜厚度、胸水消失时间及治疗前后两组患者的生活质量改善情况。结果 观察组患者治疗有效率高于对照组,不良发生率低于对照组（P〈0.05）;治疗后观察组胸水排除总量多于对照组,胸膜厚度、胸水消失时间少于对照组,FEV1及FVC情况优于对照组（P〈0.05）;治疗前两组患者生活质量比较,差异无统计学意义（P〉0.05）;治疗后两组患者生活质量均得到提高,且观察组生活质量改善优于对照组（P〈0.05）。结论 地塞米松联合尿激酶治疗结核性胸膜炎效果显著,可提高患者的临床症状消失时间,降低不良反应,可在临床进行推广。Objective To investigate the clinical effect of Dexamethasone combined with urokinase in the treatment of tuberculous pleurisy.Methods 50 patients with tuberculous pleurisy from January 2014 to January 2016 in our hospital were selected,and were randomly dividied into the control group and the observation group,with 25 cases in each group.The control group was given thoracic injection of Dexamethasone,and the observation group was given thoracic injection of Dexamethasone combined with urokinase.The effective rate of treatment,the incidence of adverse reactions,the total volume of pleural effusion,the thickness of pleura, the disappearing time of pleural effusion and the quality of life of the patients before and after treatment were compared between the two groups.Results The effective rate of treatment in the observation group was higher than that of control group,and the incidence of adverse events was lower than that of control group（P〈0.05）;the total volume of pleural effusion in the observation group was more than that of control group after the treatment,and the thickness of pleura and disappearing time of pleural effusion were less than those of control group.FEV1 and FVC were better than those of control group（P〈0.05）;there was no significant difference in the quality of life between the two groups before treatment（P〉0.05）;after treatment,the quality of life in the two groups of patients were improved,and the improvement of life of quality in the observation group was better than that of control group（P〈0.05）.Conclusion Dexamethasone combined with urokinase in the treatment of tuberculous pleurisy has significant effects,and is able to improve the disappearing time of patient′s clinical symptoms,and reduce adverse reactions,which is worthy of clinical promotion.
