摘  要:目的探讨超声在胎儿小脑延髓池增宽疾病检查中的应用价值,有效提高孕妇妊娠期胎儿中枢神经系统畸形疾病检出率。方法选取我院2010年1月至2016年2月收治的经初步诊断怀疑为胎儿小脑延髓池增宽的孕妇120例,按照检查方式的不同,平均分为对照组和观察组。对照组采用超声技术进行检查,观察组采用核磁共振与超声联合进行检查,分析比较检查结果与随访结果。结果 48例为单纯小脑延髓池增宽,44例随访至产前恢复正常;3例合并羊水过多引产;6例为增宽合并其他结构异常,2例为合并单脐动脉,随访至39周恢复正常;1例合并颅内肿囊引产,2例合并心内膜垫缺损引产,1例合并右脑室后角增宽,随访至39周恢复正常;5例为Dandy-walker畸形,1例为Dandy-walker变异;两组临床检查准确度比较差异无统计学意义(P>0.05)。结论临床对孕妇进行胎儿小脑延髓池检查时,超声能有效反映胎儿小脑延髓池增宽情况以及其他异常情况。Objective To explore the application value of ultrasound examination of fetal cerebellar medulla oblongata pool broading in order to effectively improve the detection rate of fetal central nervous system malformations during pregnancy. Methods 120 cases of pregnant women with fetal cerebellar medulla oblongata pool broading in primary diagnosis admitted to our hospital from January 2010 to February 2016 were selected and equally divided into control group and observation group according to the different methods of inspection. The control group was detected by ultrasonic technology, while the observation group was detected by nuclear magnetic resonance (NMR) combined with ultrasound. The resuks of examination and follow-up were compared and analyzed. Results 48 cases were isolated cerebellar medulla oblongata pool broadening and 44 cases were antenatally back to normal with follow-up. 3 cases were complicated with hydramnios induced labor. 6 cases were broadening complicated with other structural abnormalities, 2 cases were complicated with single umbilical artery, and were back to normal after 39-week follow-up. 1 case was complicated with intracranial swollen bursa induced labor, 2 cases were endocardial cushion defect induced labor and 1 case with fight ventricle angle broadening, and were back to normal after 39-week follow-up. 5 cases were Dandy-walker malformation, and 1 case was Dandy-walker variations. No statistical difference was found in the clinical examination accuracy between two groups (P〉0.05). Conclusions The clinical examination of the fetal cerebellum medulla oblongata pool for the pregnant woman, ultrasound can effectively reflect the condition of fetal cerebellum medulla oblongata pool broadening and other anomalies.
