腹腔镜胆囊切除胆道探查术治疗胆囊结石合并胆总管结石的效果研究

摘　　要：目的对比与分析腹腔镜胆囊切除胆道探查术与传统开腹手术在胆囊结石合并胆总管结石治疗中的临床应用疗效。方法选取2013年1月至2016年1我院收治的胆囊结石合并胆总管结石患者80例分为观察组(48例)和对照组(32例)。观察组行腹腔镜胆囊切除胆道探查术,对照组行传统开腹手术治疗。对比两组手术治疗效果、手术时间、术中出血量、术后通气时间、住院时间以及并发症发生率。结果观察组的治疗总有效率为97.9%,明显高于对照组的75.0%(P<0.05);观察组的手术时间、术中出血量、术后通气时间和住院时间均少于对照组(P<0.05);观察组的并发症发生率为2.1%,明显低于对照组的21.9%(P<0.05)。结论在胆囊结石合并胆总管结石的治疗中,相比于传统开腹手术,腹腔镜胆囊切除胆道探查术的疗效确切,创伤小,而且能够缩短患者康复时间。Objective To compare and analyze the clinical application effect of laparoscopic cholecystectomy combined with bile duct exploration and traditional open surgery in the treatment of gallbladder stone complicated with common bile duct stone. Methods 80 cases of patients with gallbladder stone complicated with common bile duct stone admitted to our hospital from January 2013 to .January 2016 were selected and divided into observation group （48 cases） and control group （32 cases）. The observation group received laparoscopic cholecystectomy combined with bile duct exploration, while the control group received traditional open surgery. The surgical effect, operation time, intraoperative blood loss, postoperative ventilation time, hospitalization time and incidence of complications were compared between two groups. Results The total effective rate of observation group was 97.9%, significantly higher than 75.0% of control group （P 〈0.05）. The operation time, intraoperative blood loss, postoperative ventilation time and hospitalization time of observation group were less than those of control group （P〈0.05）. The incidence of complications of the observation group was 2.1%, significantly lower than 21.9% of control group （P 〈0.05）. Conclusions In the treatment of gallbladder stone complicated with common bile duct stone, laparoscopic cholecystectomy combined with bile duct exploration, compared with traditional open surgery, has effective effect and minimal invasion, which can shorten the recovery time of patients.
