PPH术与痔外剥内扎术治疗妊娠中、晚期内痔疗效的比较研究

摘　　要：目的探讨吻合器痔上黏膜环切术(PPH)与痔外剥内扎术在妊娠中、晚期内痔的治疗效果。方法选取我院2014年3月至2016年2月收治的妊娠中、晚期内痔患者60例,随机分为两组,各30例。研究组采用PPH术,对照组采用痔外剥内扎术。比较两组患者的手术时间、术中出血量、术后疼痛、住院时间及术后并发症发生率、对母体胎儿的影响。结果研究组的手术时间、术中出血量、术后疼痛评分和住院时间均低于对照组,差异具有统计学意义(P<0.05)。研究组的术后尿潴留、感染、暂时性大便失禁发生率均低于对照组,差异具有统计学意义(P<0.05)。两组的术后胎心率异常、阴道流血、流产发生率比较差异无统计学意义(P>0.05)。结论相比于痔外剥内扎术,PPH术在妊娠中、晚期内痔的治疗具有创伤小、恢复快、并发症发生率低等优点,疗效更显著。Objective To explore the treatment effect of procedure for prolapse and hemorrhoids （PPH） and ligation surgery in the treatment of internal hemorrhoids during middle and late pregnant period. Methods 60 cases of patients with internal hemorrhoids during middle and late pregnant period admitted to our hospital from March 2014 to February 2016 were selected and randomly divided into two groups, with 30 cases in each group. The study group received PPH, while the control group received ligation surgery for internal hemorrhoids. The operation time, intraoperative blood loss, postoperative pain, hospitalization time, incidence of postoperative complications and effects on mother and fetus were compared between two groups. Results The operation time, intraoperative blood loss, postoperative pain score and hospitalization time of study group were all lower than those of control group, with statistical differences （P 〈0.05）. The incidence of postoperative urinary retention, infection and temporary fecal incontinence of study group were lower than those of control group, with statistical differences （P 〈0.05）. No statistical difference was found in the incidence of postoperative abnormal fetal heart rate, vaginal bleeding and abortion between two groups （P 〉0.05）. Conclusions Compared with the ligation surgery, PPH in the treatment of internal hemorrhoids during middle and late pregnant period has some advantages, such as less trauma, quicker recovery and lower incidence of complications, with more significant curative effect.
