摘  要:目的探讨952例面部不对称患者髁突SPECT骨显像结果及其数据特点,分析性别和年龄对结果的影响。方法952例患者进行髁突SPECT骨显像,选取核素摄取最高的三幅横断图像中髁突的ROI,测量其最大计数、平均计数,计算单侧计数与两侧计数之和的比值,数据结果进行统计学分析。结果1)通过最大计数及平均计数测得的比值,相关系数r=0.951,P=0.000〈0.01,结果有统计学意义;2)两侧髁突活性对比,右侧优势44.33%,左侧优势55.67%,χ^2=6.1458,P=0.013〈0.05,差异有统计学意义,男女对照,χ^2=1.040,P=0.308〉0.05,差异没有统计学意义;3)髁突活性比值:按男女对照,男性428例,女性524例,进行Wilcoxon秩和检验,Z=-1.819,P=0.069〉0.05,差异没有统计学意义。按年龄对照,22岁以下组711例,23岁以上组241例,Z=-2.139,P=0.032〈0.05,差异有统计学意义;4)髁突活性比值异常的比例:髁突活性比值在0.45~0.55区间以内为正常,活性异常的比例为45.06%(429/952),男性42.52%(182/428),女性47.14%(247/524),χ^2=1.8438,P=0.175〉0.05,差异没有统计学意义。22岁以下组47.26%(336/711)活性异常,23岁以上组,38.59%(93/241)活性异常,χ^2=5.1183,P=0.024〈0.05,差异有统计学意义。结论最大计数法与平均计数法测定髁突活性结果相关性高,同时本研究表明偏颌畸形患者中髁突为左侧优势型居多,髁突活性差异与性别无关但是与年龄相关。Objective To retrospectively analyze the results of condylar SPECT bone scan in 952 mandibular asymmetric cases by the characteristics of data and the effect of gender and age. Methods In 952 cases of condylar bone SPECT, the activities of 3 consecutive transversal ROIs were determined, whereby in the ROI, the mean and maximum pixel values were determined in a group of contiguous pixels. To determine the relative activity of each condylar region, the following equation was used : ( counts in ipsilateral region)/( counts in ipsilateral region + countsin contralateral region). Then the results were statistically analyzed by SPSS for Chi-square test and Wilcoxon rank sum test. Results 1 ) Maximum and mean condylar activity values were strongly correlated ( r =0.951, P =0. 000 〈0.01); 2) Left condyles were more likely to be hyperplasia (55.67% to 44.33%,χ^2= 6. 1458, P = 0.013 〈 0.05 ) , and there was no statistical significance between male and female; 3 ) There was no statistical sig- nificance in condylar activity values between male (428 cases) and female (524 cases) ( Z = -1. 819, P = 0. 069 〉 0.05 ) , but there was statistical significance between the patients under 22 years old, 711 cases) and over 23 years old (241cases) ( Z = - 2. 139, P =0. 032 〈0.05) ; 4) 45.06% (429/952) cases) of condylar activity values were abnormal under the cut-off value out of 0.45 to 0.55, 42.52% male (182/428 cases) and 47.14% female (247/524 cases). There was no statistical significance between male and female ( χ^2 = 1. 8438, P = 0. 175 〉 0.05 ) , but there was statistical significance between the patients under 22 years old (336/711 cases) and over 23 years old (93/241cases) (χ^2 =5. 1183, P =0. 024 〈0.05). Conclusion Maximum and mean condylar activity values are highly correlated in patients with mandibular asymmetry.
