摘　　要：目的:对比本院生殖中心层流洁净手术室改进前后体外受精-胚胎移植结局的差异,以期为临床工作提供理论依据。方法:层流洁净手术室改进前3267个生殖周期为A组,改进后的3370个生殖周期为B组。按本中心常规辅助生殖方案进行体外受精和胚胎移植,分析两组的年龄、不孕年限、受精率、2PN卵裂率、优质胚胎率、种植率、临床妊娠率等指标的差异。结果:A组受精率、卵裂率、优质胚胎率、种植率、临床妊娠率与B组比较均有统计学意义(P<0.05),两组多胎率和流产率差异无统计学意义(P>0.05)。校正年龄与不孕年限的影响后,B组受精率、卵裂率、优质胚胎率、种植率、临床妊娠率和多胎率仍显著高于A组(P<0.05),流产率低于A组(P<0.05)。结论:层流洁净手术室的建立对于提高临床妊娠结局具有重要意义,值得在生殖中心和医院中推广。Objective： To compare the outcome of IVF-ET before and after improving the environment of laminar flow surgery room. Method： The data of 6637 fresh IVF/ICSI-ET cycles from 2011 to 2015 were retrospectively analyzed. The cycles were divided into group A （before improving environment, n=3267） and group B （after improving environ- ment, n= 3370）. The pregnancy outcomes including age, infertility duration, fertilization rate, cleavage rate, good quality embryo rate, implantation rate, and clinical pregnancy rate were compared between the two groups. Result： The fertilization rate, cleavage rate, good quality embryo rate, implantation rate, and pregnancy rate in group B was significantly higher than those in group A （P〈0.05）, but the multiple pregnancy rate and abortion rate were not sig- nificantly different （P〈0.05）. After adjusting the effect of age and infertility duration, the rates of fertilization, cleaw age, good quality embryo, implantation, clinical pregnancy and multiple pregnancy in group B were significantly higher than those in group A, while abortion rate in group B was significantly lower than that in group A （P〈0.05）. Conclu- sion： Improvement of the environment of laminar flow surgery room can significantly improve the outcomes of IVF- ET, which is worth of promotion in other assisted reproductive technology center.
