非寄生虫性脾囊肿19例手术治疗经验

摘　　要：目的分析非寄生虫性脾囊肿患者外科手术疗效，探讨其最佳治疗策略。方法根据相关纳入标准，回顾性分析北京朝阳医院2008年4月至2016年4月收治的19例行手术治疗的非寄生虫性脾囊肿患者的临床资料。分析其围手术期安全性和手术治疗效果。依据手术方式的不同将患者分为腹腔镜组和开腹组，比较两组患者的相关资料。结果所有患者均顺利完成手术，10例患者行脾脏部分切除术，8例患者行脾脏全切术，1例患者行脾囊肿开窗术；13例患者行开腹手术，6例患者行腹腔镜手术，无围手术期死亡患者。脾囊肿直径平均为（6．9±2．9）cm。术后2例患者发生并发症，包括1例伤口感染，1例淋巴瘘，均经保守治疗后痊愈。腹腔镜组患者在手术时间、术中出血量、术后排气时间和术后出院时间方面明显优于开腹组，差异有统计学意义（P〈0．05）。结论手术切除是治疗非寄生虫性脾囊肿的最佳策略；腹腔镜手术为其治疗提供了一种新的思路。Objective To analyze the results of surgical treatment on 19 patients with non-parasitic splenic cysts, with an aim to determine the best treatment strategy. Methods We retrospectively analyzed the clinical data of 19 consecutive patients with non-parasitic splenic cysts operated from April 2008 to April 2016 at the Beijing Chaoyang Hospital. We then analyzed the safety and effectiveness of surgical treatment according to the different surgical methods used. The patients were divided into the laparoscopic group and the open group, and the clinical data of the two groups were compared. Results All the patients were suc- cessfully treated by surgery, lO patients underwent partial splenectomy, 8 splenectomy and 1 splenic cyst fenestration. Thirteen patients underwent open surgery, and 6 laparoscopic surgery. No patients died in the perioperative period. The splenic cyst diameter was （6.9 ± 2.9） cm. There were 2 patients who developed postoperative complications, including 1 with wound infection and 1 with lymphatic fistula. The operation time, bleeding volume, time to first flatus and discharge time in the laparoscopic group were significantly better than the open group, and the differences were significant （P 〈 0.05）. Conclusions Surgical resection was the best strategy for treating non-parasitic splenic cysts. Laparoscopic surgery was a good treatment option.
