窄谱中波紫外线治疗对寻常型银屑病Th22细胞及其相关细胞因子表达的影响

摘　　要：目的:观察窄谱中波紫外线(NB-UVB)治疗寻常型银屑病的疗效以及对寻常型银屑病患者Th22细胞及相关细胞因子的影响,探讨NB-UVB治疗银屑病的作用机制。方法:40例寻常型银屑病患者NB-UVB治疗20次,以皮损面积和严重程度(PASI)评分评价疗效;取患者治疗前后的外周血,用三色法流式细胞仪检测Th22细胞,ELISA法检测IL-6及IL-22的水平,并与健康对照组进行比较。结果:NB-UVB治疗后PASI评分明显下降(P<0.01);银屑病患者治疗前外周血Th22、IL-6及IL-22的水平较对照组均明显增高(P<0.01)。治疗后外周血Th22细胞的百分比(1.19±0.31)%,血清IL-6的水平(15.27±1.66)pg/mL及IL-22的水平(38.52±3.31)pg/mL较治疗前均明显下降(P值均<0.01),且三者水平高低均与PASI呈正相关(P值均<0.05)。结论:NB-UVB可能通过影响Th22细胞及IL-6、IL-22细胞因子来发挥治疗银屑病的作用。Objective：To observe the therapeutic effect and the influence on Th 22 cells and re-lated cytokines in peripheral blood of psoriasis vulgaris that were treated with narrow -band ultravio-let B （ NB-UVB） , and to analyze NB-UVB＇s mechanism of action .Methods：Forty cases of psoria-sis vulgaris were treated with 20 times NB-UVB radiation .The therapeutic effect was evaluated u-sing Psoriasis Area and Severity Index （ PASI ） scores.Peripheral blood was obtained from nor-mal, healthy controls and patients with psoriasis vulgaris before and after NB-UVB radiation treat-ment.Three-color flow cytometry was carried out to detect the quantity of Th 22 cells.ELISA was used to detect levels of serum IL-6 and IL-22.Results：The mean PASI scores of patients de-creased significantly after treatment with NB-UVB radiation （ P 〈0.01 ） .Levels of Th22 , IL-6 and IL-22 in patients with psoriasis vulgaris before treatment were significantly higher than those of＆amp;nbsp;healthy controls （P〈0.01）.After treatment with NB-UVB,levels of Th22 [（1.19 ±0.31）%], IL-6 [ （ 15.27 ±1.66 ） pg/mL ） ] and IL-22 [ （ 38.52 ±3.31 ） pg/mL ] decreased significantly （ P〈0.01 ） in patients with psoriasis vulgaris , and were positively correlated with PASI （ P 〈0.05）.Conclusion：NB-UVB may play a role in the treatment of psoriasis vulgaris by affecting Th22 cells and related cytokines .
