鼻内镜联合纤维喉镜行上颌窦囊肿摘除术的疗效及对血清学指标影响的研究

摘　　要：目的:分析鼻内镜联合纤维喉镜行上颌窦囊肿摘除术的疗效及对血清学指标的影响。方法:选择106例接受上颌窦囊肿摘除术治疗的上颌窦囊肿患者,按照随机数表法将其分为观察组与对照组,每组53例。观察组接受鼻内镜联合纤维喉镜治疗,对照组接受单纯鼻内镜治疗,比较两组患者的治疗效果、炎症因子水平以及术后并发症等情况的差异。结果:观察组治疗后痊愈及好转概率显著高于对照组,两组比较有差异(x^2=7.395,x^2=6.281;P<0.05);观察组无效及复发概率低于对照组,两组比较有差异(x^2=7.793,x^2=8.039;P<0.05);观察组治疗后白介素-1(IL-1)、白介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)血清炎症因子水平明显低于对照组,两组比较有差异(t=0.142,t=0.383,t=0.227;P<0.05);观察组治疗后各项并发症发生率均明显低于对照组,两组比较有差异(x^27.395,x^2=9.173,x^2=6.039,x^2=7.193,x^2=6.395;P<0.05)。结论:鼻内镜联合纤维喉镜行上颌窦囊肿摘除术可以有效提升治疗效果,降低血清炎症因子水平,具有良好的治疗安全性。Objective： To analysis the effect of nasal endoscopy combined fiber laryngoscope on patients with maxillary sinus cysts and serological parameters changes. Methods：106 cases of patients with maxillary sinus cysts between June 2012 to June 2014 in our hospital were treated with maxillary sinus cyst excision, and were divided into the observation group （53 cases） treated with nasal endoscope combined fiber laryngoscope and the control group （53 cases） treated with pure nasal endoscopic treatment according to the random indicator method. Then curative effect, inflammatory factors level and postoperative complications were compared. Results：1） The patients in the observation group healing and improving probability were significantly higher than the control group, and patients with invalid and recurrent risk were lower than the control group （x2=7.395, x2=6.281;P〈0.05）;2） The interleukin-1 （IL-1）, interleukin-8 （IL-8）, tumor necrosis factor-α（TNF-α） and serum inflammatory factors level in the observation group was significantly lower than that in the control group （t=0.142, t=0.383, t=0.227;P〈0.05）;3） The complications in the observation group were significantly lower than that in the control group （x2=7.395, x2=9.173, x2=6.039, x2=7.193, x2=6.395; P〈0.05）. Conclusion： Nasal endoscopy combined fiber laryngoscope on maxillary sinus cyst excision can enhance treatment effect, reduce serum levels of inflammatory factors and be a good security for treatment.
