癌症病人濒死期护理路径在临终关怀病房的应用研究

摘　　要：[目的]探讨癌症病人濒死期护理路径在临终关怀病房应用的效果。[方法]本研究为非平行对照试验,在路径实施前将符合纳入标准的40例病人列为对照组,进行常规护理;路径组40例病人则使用濒死期护理路径进行管理。4h记录1次病人的疼痛、呼吸困难、恶心呕吐、分泌物梗阻、烦躁不安5种症状的发生情况,并在病人死亡后由家属填写护理质量评价量表。[结果]路径组病人5种症状的发生率均低于对照组,在疼痛、呼吸困难方面差异有统计学意义(P<0.05),以4h记录1次的总记录次数症状发生率在疼痛、呼吸困难、分泌物梗阻方面差异有统计学意义(P<0.05),且路径组病人的临终护理质量得分高于对照组(P<0.05)。[结论]癌症病人濒死期护理路径的实施能够控制濒死期癌症病人症状,改善晚期癌症病人临终生存质量,提高整体照顾质量。Objective：To probe into the application effect of nursing pathway of cancer patients in agonal stage in hospice care unit .Methods：The study was a non parallel controlled trial .A total of 40 patients who met the in-clusion criteria before the implementation of the pathway were taken as control group ,and they received routine nursing ;while another 40 patients in pathway group received agonal nursing path management .Occurrence situ-ation of 5 kinds of symptoms including pain ,dyspnea ,nausea and vomiting ,discharge obstruction ,irritability of patients were recorded per 4 h ,and the scale of care quality evaluation was filled in after they died by their fami-ly members .Results：Incidence of 5 kinds of symptoms in pathway group was significantly lower than that in control group .There was statistically significant difference in pain and dyspnea （P〈0 .05） .There was statisti-cally significant differences in total recorded number of symptoms per 4 h including incidence of pain ,dyspnea , discharge obstruction（P〈0 .05） ,and the score of hospice nursing quality in pathway group was higher than that in control group （P〈 0 .05） .Conclusion：The implement of nursing pathway of cancer patients in agonal stage could control symptoms of cancer patients in agonal stage ,improve quality of hospice life of advanced cancer patients and enhance the overall quality of care .
