血液NT-proBNP、CTnT和CA125在慢性心力衰竭的应用研究

摘　　要：目的探讨血清N末端B型利钠肽原(NT-proBNP)、心肌肌钙蛋白T(CTn T)和血浆糖类抗原125(CA125)在慢性心力衰竭疾病中的应用。方法将慢性心力衰竭患者107例作为观察组,选择同期健康体检者43例作为正常对照组。采用电化学发光免疫法检测血清NT-proBNP、CTn T和血浆CA125,比较观察组、对照组其在不同NYHA分级中三项指标的表达水平,观察三项指标单项检测及联合检测的灵敏度及特异度。结果观察组血清NT-proBNP、CTn T和血浆CA125水平均明显高于正常对照组(P<0.05);不同NYHA分级血清NT-proBNP、CTn T和血浆CA125水平均明显高于正常对照组(P<0.05),随着分级上升水平越高。三项指标联合检测的灵敏度及特异度高于单项检测的灵敏度及特异度。结论血清NT-proBNP、CTn T和血浆CA125与慢性心力衰竭程度密切相关,三项指标联合检测可提高慢性心力衰竭的灵敏度及特异度,对慢性心力衰竭诊断具有重要作用,且可作为慢性心力衰竭心功能分级的评估指标。Objective To investigate the application of serum NT - proBNP, CTnT and plasma CA125 in the chronic heart failure disease. Methods 107 patients with chronic heart failure were selected as the observation group and 43 healthy subjects were re- cruited as a control group. The electro chemiluminescence immunoassay was used to detect serum NT - proBNP, CTnT and plasma CA125. The expression levels of three indicators in the NYHA classification were compared between the observation group and the con- trol group. The sensitivity and specificity of these three indicators were observed by single and joint detection. Results The level of ser- um NT - proBNP, CTnT and plasma CA125 in the observation group was significantly higher than that of the control group （P 〈 0.05 ）, and the higher of the NYHA classification, the higher was the level of those indicators. The sensitivity and specificity of the three indica- tors in joint detection were higher than those in the single detection. Conclusion Serum NT -proBNP, CTnT and plasma CA125 were closely related to the level of chronic heart failure. The joint detection could improve the sensitivity and specificity of chronic heart fail- ure. It could be used to diagnose chronic heart failure, and can be used as indicators in assessment of cardiac function classification in chronic heart failure.
