不同Child-Pugh分级的肝硬化患者凝血四项以及胆碱酯酶检测的临床意义

摘　　要：目的探讨不同Child-Pugh分级的肝硬化患者化验凝血四项以及胆碱酯酶指标变化趋势对于评估肝硬化患者的发展及预后的临床意义。方法选取该院80例不同Child-Pugh分级的肝硬化患者和20例健康体检者为研究对象,对其血清胆碱酯酶(ChE)、血浆凝血酶原时间(PT)、部分凝血活酶时间(APTT)、凝血酶时间(TT)和纤维蛋白原(FIB)的检测结果进行对照分析。结果不同Child-Pugh分级的肝硬化患者PT、APTT和TT明显长于健康对照组(P<0.05);不同Child-Pugh分级的肝硬化患者FIB、Ch E显著小于健康对照组(P<0.05);不同Child-Pugh级别肝硬化患者PT、APTT、TT、FIB和Ch E值比较差异均有统计学意义(P<0.05);不同Child-Pugh级别肝硬化患者PT、APTT、TT、FIB和Ch E值两两比较差异均有统计学意义(P<0.05)。结论检测凝血四项、胆碱酯酶是判断肝硬化病情严重度的有效指标,对于不同Child-Pugh分级的肝硬化的诊断和预后判断有重要临床意义。【Objective】To explore the clinical significance of the four coagulation indexes and cholinesterase trend in the evaluation of the patients with liver cirrhosis.【Methods】This study compares the indexes of serum cholinesterase（ChE）, prothrombin time（PT）, activated partial thromboplastin time（APTT）, thrombin time（TT） and fi brinogen（FIB） collected from 80 patients of liver cirrhosis and 20 healthy check-ups respectively.【Results】When compared with the healthy checkups, the indexes of PT, APTT, TT of the patients with liver cirrhosis at different Child-Pugh grades were gradually prolonged（P〈0.05）; FIB and Ch E of the patients with liver cirrhosis at different Child-Pugh grades reduced（P 0.05）.【Conclusion】The four coagulation indexes and cholinesterase as the effective signs of liver cirrhosis level at different Child-Pugh grades are of great clinical signifi cance for diagnosis and prognosis of liver cirrhosis.
