摘 要:目的测试自主研发的自体骨腰椎椎间融合器的生物力学稳定性。方法将12具猪腰椎标本随机分为两组:(1)实验组:自体骨腰椎椎间融合器联合椎弓根螺钉内固定组;(2)对照组:PEEK cage联合椎弓根螺钉内固定组。在正常状态、不稳定状态、固定状态下对2组标本进行前屈、后伸、左右侧弯、轴向旋转6个方向的生物力学测试,并对数据进行分析。结果正常状态下,2组间ROM数据差异无显著性意义(P>0.05);不稳状态下,2组ROM数据较正常状态明显增加(P<0.05);固定融合状态下,两组ROM数据较不稳状态下明显减小(P<0.05),两组均能明显提高脊柱的即刻生物力学稳定性,而且两组间结果无显著性差异(P>0.05)。结论自体骨腰椎椎间融合器和PEEK cage均能满足腰椎力学即刻稳定性要求。Objective To examine a autogenous bone lumbar intervertebral fusion cage immediate biome- chanical stability. Methods The autologous bone lumbar intervertebral fusion cage was made by some equip- ment. Resin embedding method and graded facetectomy and discectomy were made to prepare spinal instability models. Experimental group used autogenous bone forming lumbar intervertebral fusion cage with vertebral pedi- cle screw fixation; control group used cage with vertebral pedicle screw fixation. The three - dimensional motion of specimens was tested in intacted state, instability state and fusion state, which the flexion, extension, bilat-eral bending and axial rotation loading were applied to the specimens and the range of motion (ROM) were de- termined by the three - dimensional laser scanner. Results There was no significant difference in all stability in- dexes (ROM) between two groups in intact state(P 〉 0.05). Compared with the intact state, there was signifi- cant difference in all stability indexes in instable state between the two groups. After posterior lumbar interbody fusion, there was no significant difference in all stability indexes between two groups(P 〈 0.05). Both the auto- genous bone forming lumbar intervertebral fusion cage with vertebral pedicle screw fixation and cage with verte- bral pedicle screw fixation can improve the immediate biomechanical stability of the spine, and there is no statis- tical difference in spinal stability capacity between two groups. Conehtsion The autogenous bone forming lum- bar intervertebral fusion cage can provide the good immediate biomechanical stability.
