17G同轴套管活检枪在肺亚厘米结节CT引导下穿刺活检中的应用

摘　　要：目的探讨在CT引导下应用17G同轴套管活检枪对肺内亚厘米结节进行穿刺活检的技术技巧及价值。方法回顾性分析总结24例直径在0．6～1．0cm肺内亚厘米结节病变应用17G同轴套管活检枪进行CT引导下穿刺活检的技术技巧及准确率、并发症。结果24例患者在充分完善的术前准备下，术中均顺利完成活检术，穿刺活检成功率为100％，标本特异性为100％，气胸、胸痛、咳血及穿刺针道少量出血发生率为25％、zo％、25％、87．5％。结论在CT引导下应用17G同轴套管活检枪对肺内亚厘米结节进行穿刺活检，具有较高的技术技巧含量，其明显提高了穿刺成功率，准确率高，临床应用价值大，值得推广应用。Objective To analyze the technical skills and value of puncture biopsy of sub-centimeter pulmonary nodule under CT guided by using 17G coaxial biopsy gun. Methods The technical skills and accuracy rate,complications of 24 cases of pathologically confirmed sub-centimeter pulmonary nodules which diameterswere between 0.6- 1.0 cm under CT guided were collected and retro- spectively reviewed. Results Under fully perfect preoperative preparation,CT puncture biopsy were underwent successfully in 24 patients. Biopsy success rate was 100%, specimen specificity was 100%. The rate of pneumothorax, chest pain, hemoptysis, scarce needle tract bleeding were 25% ,20%,25 %,87.5% respectively. Conclusion Biopsy success rate of pulmonary sub-centimeter nod- ules is obviously improved with using 17G coaxial biopsy gun under CT guiding,the accuracy is high,at the same time this operation needs higher technical skills and abilities,it has the huge value of clinical application which should be widely applied.
