功能性鼻窦内窥镜术后联合采用缓冲高渗盐溶液鼻腔冲洗对变应性鼻-鼻窦炎患者血清中IL-6、IL-8及IL-10的影响

摘　　要：目的探讨功能性鼻窦内窥镜术后联合采用缓冲高渗盐溶液鼻腔冲洗对变应性鼻-鼻窦炎患者血清中炎性因子(IL-6、IL-8及IL-10)的影响。方法将70例ARS患者按照治疗方法随机分为对照组与治疗组,各35例。治疗组患者术后给予缓冲高渗盐溶液对鼻腔进行冲洗,再使用丙酸氟替卡松喷雾剂进行喷鼻;对照组患者在FESS术后采用鼻腔内丙酸氟替卡松鼻喷雾剂进行喷鼻处理。比较2组临床疗效、治疗前后临床症状及体征评分、治疗前后RQLQ量表评分、治疗前后血清IL-6、IL-8及IL-10水平。结果观察组临床总有效率(94.29%)显著高于对照组(65.71%);2组患者治疗后血清IL-6及IL-8水平均显著低于治疗前,血清IL-10水平显著高于治疗前,差异均有统计学意义(P<0.05);且2组患者治疗后血清IL-6、IL-8及IL-10水平差异均有统计学意义(P<0.05)。结论FESS术后联合采用缓冲高渗盐溶液鼻腔冲洗可有效降低患者血清IL-6、IL-8水平,提高IL-10水平,疗效显著,可在临床上推广使用。Objective To discuss the effect of functional endoscopic sinus surgery combined with use of buffered hypertonic saline nasal allergy nasal irrigation solution- sinusitis on the serum inflammatory factors（ IL- 6,IL- 8 and IL- 10）. Methods 70 ARS patients,according to different treatments,were randomly divided into the control group and the treatment group,with 35 patients in each group. The treatment group was given a solution buffered hypertonic saline nasal rinse after surgery,and then nebulized fluticasone propionate nasal spray; while the control group was,after FESS,given intranasal fluticasone propionate nasal spray nasal spray. Comparison of the clinical efficacy of the two groups,clinical symptoms and signs scores before and after treatment,RQLQ scale score,serum IL- 6,IL- 8 and IL- 10 level before and after treatment was conducted. Results Clinical total effective rate of the observation group（ 94. 29%） was significantly higher than the control group（ 65. 71%）; serum IL- 6 and IL- 8 levels between the 2 groups after treatment were significantly lower than before the treatment,while IL- 10 was higher than before the treatment,and the differences were statistically significant（ P〈0. 05）; and differences of serum IL- 6,IL- 8 and IL- 10 levels between the two groups after treatment were statistically significant（ P〈0. 05）. Conclusion Postoperative FESS using buffered hypertonic saline nasal irrigation can effectively reduce serum IL- 6,IL- 8 levels and increase IL- 10 level,which has significant effect and can promote the clinical application.
