医院-社区-家庭管理模式在农村社区老年人院前压疮管理中的应用

摘　　要：目的探讨降低农村社区卧床老年人院前压疮发生率的方法。方法通过建立医院-社区-家庭管理模式对农村社区居民重点针对压疮高危老年人家属及照护者进行压疮预防护理的干预,比较建立医院-社区-家庭管理模式前后压疮高危老年人照护者的压疮知识掌握情况、社区老年人院前压疮发生情况及院前压疮发生的严重度。结果建立医院-社区-家庭管理模式后,压疮高危老年人照护者的压疮知识掌握情况明显优于模式建立前(P<0.01);压疮发生率明显下降(P<0.01)。结论建立医院-社区-家庭管理模式可提高照护者的压疮知识掌握情况及降低社区压疮高危老年人的院前压疮发生率。Objective To explore approaches to control the incidence of pressure ulcer among bedridden elderly patients in rural communities. Method Hospital - community - family management model was implemented to educate families and （or） caregivers of pressure ulcer high - risk elderly about preventive measures of pressure ulcers. Caregivers knowledge of pressure ulcer and the incidence and severity of pre - hospital pressure ulcers in community elder- ly were compared before and after intervention. Result Since hospital - community - family management model set up, the knowledge of pressure ulcer of pres- sure ulcer high - risk elderly caregivers improved （ P 〈 O. O1 ）, and the incidence of pre - hospital pressure ulcer among the high - risk elderly decreased （ P 〈 0. 0l ）. Conclusion Hospital - community - family management model helps to improve caregivers＇ knowledge about pressure ulcer and reduce pre - hos- pital pressure ulcer incidence rate in high - risk community elderly.
