摘  要:目的:探讨多层螺旋CT容积扫描在颈椎病中医分型中的临床应用价值。方法:回顾性分析51例颈椎病患者的中医分型与CT表现,行颈椎螺旋模式扫描,模拟重建常规单层C2/3、C3/4、C4/5、C5/6、C6/7椎间盘层面。容积扫描参数为120 KV,300 MAS,层厚0.75 mm,扫描范围约108~168 mm,探讨颈椎容积扫描及MPR重建对颈椎病中医分型的优越性。结果:颈椎病中医分型与CT影像表现如下,风寒湿型8例,颈部生理曲度变直或反弓,项韧带钙化,椎小关节增生;气滞血瘀型12例,间盘突出,椎间孔狭窄,神经根受压;痰湿阻络型20例,椎间隙变窄,椎间盘突出,椎管狭窄,横突孔狭窄,椎体骨质增生;肝肾不足型6例,椎间盘突出,脊髓受压变窄弯曲移位;气血虚亏型5例,椎小关节对位不良,关节间隙变窄,间盘变性,椎体失稳。结论:颈椎容积(VH)扫描MPR重建能显示更多的征象,明确病变的部位、范围,对于颈椎椎管的骨性椎管狭窄,椎间孔、横突孔的狭窄,椎间隙变窄,椎间盘突出,关节突关节退行性改变,后纵韧带钙化,项韧带钙化的显示率提高明显,为颈椎病中医分型提供了客观指标,有助于提高颈椎病中医诊疗效果。Objective:To evaluate the clinical application of muti-slice helical CT volumetric(VH)scanning in vertebrae cervicales disease of traditional Chinese medicine(TCM)Types. Methods:51 cases with vertebrae cervicales disease of TCM types and CT manifestations were retrospectively analyzed by the VH scanning mode of cervical spine,simulated the traditional protocol of single-slice(SS),discrete scanning for C2/3,C3/4,C4/5,C5/6,C6/7 intervertebral discs. Volume scanning parameters was performed with 120 KV,300 MAS,collimation of 0.75 mm,the scans covered from 108 mm to 168 mm. Discussion on the superiority of cervical vertebra volume scan and MPR reconstruction in TCM types of vertebrae cervicales disease. Results:Vertebrae cervicales disease of TCM types and CT imaging appearances were as follows. 8 cases of wind cold dampnes showed straight or reverse arch of the cervical physiological curvature,nuchal ligament calcification and hyperosteogeny of vertebral facet joint. 12 cases of Qi stagnation and blood stasis showed cervical disc protrusion,intervertebral foramina stricture and nerve root compression. 20 cases of phlegm-dampness retention showed cervical intervertebral space stenosis,herniated cervical intervertebral discs,spinal canal stenosis,transverse foramen stenosis and vertebral bone hyperplasia. 6 cases of liver and kidney deficiency showed herniated cervical intervertebral discs and spinal cord compression. 5 cases of Qi-blood deficiency showed vertebral facet joint dislocation,cervical intervertebral space stenosis,cervical intervertebral disc degeneration and cervical spine instability. Conclusion:The VH scanning and MPR reconstruction mode of cervical spine showed far more features to define the location and extent of lesion and to significantly improve displaying rate of bony narrowing of the cervical vertebral canal,the intervertebral foramina and transverse foramen stenosis,cervical intervertebral space stenosis,herniated cervical intervertebral discs,the facet joint degenerative,c
